The Opportunity:

We are seeking an Evergreen Care Management Specialist (HOPE Program) to support justice-involved youth through holistic, wraparound case management services. Reporting to the Evergreen Care Management Supervisor, this role assists youth on their journey toward life stabilization by coordinating internal and external referrals, supporting justice system navigation, and fostering meaningful community partnerships. The successful candidate will provide structured case management, facilitate workshops and outreach initiatives, and contribute to a supportive, faith-based environment grounded in YSM’s mission, values, and Biblical foundations.

Responsibilities:

Direct Care Management (Justice-Involved Youth)

  • Provide individualized, trauma-informed case management to justice-involved youth using a holistic wraparound approach

  • Deliver community-based support by meeting youth in homes, community spaces, partner agencies, and other accessible locations

  • Support youth in navigating the criminal justice system, including legal aid access, court preparation, disclosures, and court accompaniment

  • Develop and implement individualized service plans focused on stabilization, reintegration, and personal development

  • Coordinate internal and external referrals for mental health, housing, employment, healthcare, legal supports, and additional services

  • Collaborate with internal YSM teams to ensure continuity of care and coordinated service delivery

  • Maintain accurate case notes, service plans, documentation, and data entry within organizational systems (e.g., TIMES, Google Drive)

  • Provide advocacy and accompaniment to appointments, meetings, and case conferences

  • Manage client intakes and maintain an active caseload

Workshop Development and Coordination

  • Assess the needs of justice-involved youth participating in probation, diversion, and reintegration pathways

  • Design and facilitate workshops focused on emotional regulation, conflict resolution, life skills, employment readiness, restorative practices, and decision-making

  • Coordinate internal and external programming opportunities for youth

  • Track attendance and participation data through Salesforce and related reporting systems

Justice-Focused Outreach and Partnership Development

  • Build and maintain relationships with justice-sector stakeholders, including courts, probation services, legal aid, bail programs, and community agencies

  • Conduct outreach to identify and engage justice-involved youth who may benefit from HOPE Program services

  • Promote awareness of YSM and the HOPE Program through presentations, community events, and information sessions

  • Maintain a visible presence within the Ontario Court of Justice Bail Program to strengthen referral pathways

  • Attend community and justice-sector network meetings to identify trends, service gaps, and opportunities for collaboration

Qualifications:

  • Post-secondary education in Social Work, Child & Youth Care, Community Services, Psychology, Criminology, or a related field
  • Minimum 2–3 years of experience in case management, youth work, justice services, community outreach, or a related social services environment
  • Experience supporting justice-involved youth and navigating community and legal support systems
  • Strong understanding of trauma-informed, strengths-based, and anti-oppressive practices
  • Excellent interpersonal, crisis intervention, advocacy, and conflict-resolution skills
  • Strong organizational, administrative, and time-management abilities
  • Experience facilitating workshops, group programming, or youth engagement initiatives
  • Ability to work independently while collaborating within a multidisciplinary team
  • Comfortable working flexible schedules and within community-based settings
  • Proficiency with case management documentation, data entry systems, and Microsoft/Google Office platforms
  • CPI/de-escalation training or willingness to obtain training is considered an asset
  • Demonstrated personal commitment to and understanding of the mission and values of YSM

Compensation: YSM offers market competitive rates and will pay a starting salary of $54,016.58 for this role, combined with a comprehensive suite of benefits and perks.
