Social Worker - C5 Trauma - Temporary Full-Time 2026-16593 (2026-16593)
About the role
Summary Of Duties To provide a full range of social work services to patients and their families admitted to the Trauma program. This includes psycho-social assessments, counselling to patients and families, discharge planning, crisis intervention, linkage to community resources, team consultation.
Qualifications/Skills
- Masters of Social Work Degree Required
- Member of the Ontario College of Social Workers and Social Service Workers in Good Standing
- Minimum two (2) years of experience working in a Hospital setting
- Experience in Trauma preferred
- Exceptional communication skills required
- Knowledge of the HCCA and SDM act
- In-depth understanding of the Sunnybrook Policies that impact transitional planning
- Strong negotiation and conflict resolution skills
- Demonstrated skills in Interprofessional Collaborative Practice & Education
- Ability to balance collaborative practice with self-direction
- Ability to manage multiple tasks and set priorities in a fast-paced environment
- Commitment to student education
- Experience and commitment to quality improvement initiatives
- Demonstrated knowledge of organizational priorities and social work scope of practice priorities
