About the role

0.8 FTE or 31 hours /per week Position Summary: The Primary Care Registered Nurse (PCRN) will work as a part of a multidisciplinary primary care team including allied health professionals and community partners to deliver wholistic patient centered care, with consideration of all social determinants of health. The PCRN provides comprehensive team-based care to patients across the lifespan, in the patients’ medical home and at Calgary Rural PCN central clinic locations. The PCRN may support a variety of programs and services, while exercising a high degree of critical thinking, in an autonomous but collaborative and supportive environment.

Key Responsibilities:

  • Assess and screen patients for physical, and psychosocial conditions that impact health outcomes.
  • Case management of patients with highly complex needs.
  • Empower patients to manage their symptoms of a long-term disease.
  • Provide education and guidance to patients with/ or at risk for a wide range of chronic or complex health conditions commonly encountered in primary care settings.
  • Assess screening needs, promote and complete screening as per guidelines.
  • Establish patient centered SMART goals through motivational interviewing, and coaching.
  • Collaborate to enact patient centered care plans to support patients in their health journey, including facilitating goals of care discussions.
  • Determine and make appropriate referrals to internal and external providers and services.
  • Maintain accurate and timely documentation in the electronic medical record (EMR).
  • Communicate essential information in a timely manner to support seamless care.
  • Facilitate and evaluate group programs and education sessions.
  • Complete daily data entry into CRPCN data collection tool.
  • Participate and/or lead CRPCN quality improvement initiatives, evaluations / surveys as required.
  • Mentor and orientate new staff to program/service delivery.
  • Adhere to all CRPCN policies and procedures.
  • Support Goals of Care Discussions
  • Commitment to enhance educator competencies and share knowledge to support excellence in specialized care.
  • Other duties as required within Registered Nurse scope of practice.
  • Comply with all CRNA professional standards of practice including maintaining patient confidentiality

Educational Requirements:

  • Baccalaureate degree in nursing preferred or diploma in registered nursing with 5+years.
  • Active CRNA Registration in good standing.
  • Current BCLS certification for health care providers.
  • Criminal background check inclusive of vulnerable sectors check within the last 6 months.
  • Health Change Methodology or equivalent motivational interviewing education an asset
  • One or more of the following certifications strongly preferred:
  • Certified Diabetes Educator (CDE)
  • Certified Bariatric Educator (CBE)
  • Certified Respiratory Educator (CRE)
  • Team Primary Care Nurse- Post Licensure Program certification an asset.

Experience and Skills Required:

  • Minimum three years of nursing experience, preferably in a primary care setting.
  • Experience working within a multidisciplinary primary care team model.
  • Knowledge of current best practice guidelines for conditions frequently seen in a primary care setting.
  • Experience educating patients using the principles of motivational interviewing to set patient centered goals.
  • Knowledge of evidence-based health promotion, prevention, and management strategies

  • Valid driver’s license & access to reliable vehicle required.

About the Calgary Rural Primary Care Network (CRPCN): The Calgary Rural PCN collaborates with communities, family doctors and other health team members to do whatever we can help you be as healthy as possible. We are working together to change the delivery of health care in our communities from reactive to preventative by providing services to patients that fit their needs through supports we have in our member clinics, such as nurses, dietitians, and other health care professionals. We collaborate to provide the tools and education to empower patients to work on their everyday health needs.

The Calgary Rural Primary Care Network (CRPCN) is a group of family doctors and an extended team of nurse practitioners, nurses, dietitians, and mental health support all working together to provide primary care services in rural communities south, east and west of Calgary.

The CRPCN consists of 138 family doctors, 39 member clinics and 3 CRPCN clinics, provide “Medical Homes” to our patients. Patients who have more complex health issues work with PCN health team members to help them meet their heath goals and lead a healthy life.
