A****BOUT THE ROLE

We are looking for a passionate and caring Social Worker to join our multidisciplinary health care team! Together with the Community Health & Wellness team the Social Worker will work to create a system of supports for children and families. The social worker will utilize best practice and brief evidence-based treatment interventions, including psychoeducation and cognitive behavioral strategies to develop family centered behavioral care plans. This can include utilizing and creating one-on-one supports and plans for families as well as group services tailored to support the needs of children. This is a full-time regular position located in Fort McKay, AB.

KEY RESPONSIBILITIES

Primary responsibilities include, but are not limited to:

  • Adhere to organized, evidence-based models of care, and works with the team to assess, treat, and support client and families with concerns by developing goal-directed, family-centered care plans to address behavioral concerns, social/emotional challenges and overall barriers to wellness.
  • Regularly monitor reviews, evaluate, and adjust the clients’ care plan based on effectiveness of interventions and/or changes in condition or environment.
  • Identify client and family concerns rapidly and uses appropriate assessment approaches and tools to implement strategies, provide psychoeducation, and referrals for clients as needed.
  • Support child and families in navigating services, while maintaining principles of information, relational, and management continuity.
  • Acquire and maintain a comprehensive understanding of community services and referral processes, including any services, specialists, hospital care, rehabilitation, and support programs and educational programs.
  • Work collaboratively with the team in scheduling appointments and arranging follow-ups and consultations, while continuously striving toward timely access to care for clients.
  • Work in collaboration with the Community Health and Wellness team to develop panel identification strategies to detect clients “at risk” for physical or psychological deterioration.
  • Accurately record client encounters while adhering to standardized measures and guidelines.
  • Work in a flexible, responsive manner to provide social work services in a variety of practice environments within the nation.
  • Assist in the development and facilitation of social/emotional group education sessions.
  • Participate in clinical circles of care as required.
  • Work collaboratively within Community Health & Wellness to enhance the delivery of primary care services.
  • Understand confidentiality and privacy for clients seeking services
  • Review, evaluate, and document client progress and make changes to treatment plan as necessary
  • Maintain a high degree of collaboration and excellent communication with other allied service providers, case team members and family members
  • Adhere to ethical standards and regulations set by the Alberta College of Social Workers (ACSW)
  • Other related duties as assigned.

KEY REQUIREMENTS

  • Bachelor of Social Work Degree is required

  • Registration with the Alberta College of Social Workers

  • Up-to-date professional liability insurance

  • Minimum of one (1) to two (2) years of related experience. Experience in youth, crisis, trauma, anxiety is an asset; strong experience in a mental health

  • Current CPR-BLS certification

  • Experience working in a First Nations community setting including knowledge of First Nations culture, traditions and values considered an asset

  • Valid class 5 Driver’s License & Clean Driver’s Abstract

  • A clean criminal record and vulnerable sector check is required

  • Successful candidate will be required to live in (or relocate to) Fort McMurray, Alberta
