What we are looking for:
Reporting to the Director of Medicine, this position is responsible for the provision of comprehensive, specialized care for patients requiring wound, skin or ostomy care and assisting in program planning/evaluation of pressure injury prevention/management/education of front line staff. May travel to Uxbridge and Reactivation Care Centre locations when required.
Primary Responsibilities:

Build capacity for the delivery of excellent skin and wound care

Practice within the inter professional team model

Responsible for promoting best practices in skin and wound care

Facilitates application of relevant information to make effective judgments and decisions that lead the management of care

Promotes professional practice and excellence in the care of persons with skin, wound, and ostomy needs

Commitment to continuing professional development

Decision making, problem recognition and problem solving

Facilitate the identification and development of staffs learning needs through coaching and leadership, implements and evaluates strategies to increase learning and ongoing development of the inter professional team

Research, design, implement, and evaluate programs of evidence based practice

Create capacity among inter professional team members to provide excellence in skin wound and ostomy care and the evaluation of care

Provides clinical consultation to staff, patients and, or family member
-
Identifies learning needs and uses adult education concepts in planning, designing and implementing educational strategies.

Acts as a resource, coach and mentor to clinicians and opportunities for patient education

Support innovative development/implementation of patient centered approach to pressure injury management
What you bring to the role:
- Registered Nurse with the College of Nurses of Ontario.
- Master’s Degree in Nursing or working towards required
- Five years RN experience, or an equivalent combination of post graduate training specific to wound care treatment and management
- Wounds Canada membership an asset
- Completion of course work relevant to area of expertise, such as credentials for Enterostomal Therapist, International Interdisciplinary Wound Care Course or comparable course an asset
- Advanced clinical knowledge and implementation of evidenced based practice
- A strong desire and capacity to promote professional practice and excellence in evidence-based, patient centered care
- Superior interpersonal, coaching, and leadership skills
- Excellent decision making, problem recognition and problem solving skills
- Demonstrated commitment to continuing professional development
- Excellent computer literacy including knowledge of Excel
- Excellent documentation skills and data collection
- Demonstrated good attendance and performance records with the ability to maintain these same standards
- Demonstrates a commitment to a healthy and safe workplace for self and others (staff, patients, families, etc.) by taking all reasonable precautions and working in compliance with organization related policies, health and safety legislation and best practices and completing relevant mandatory education as required
Work Schedule:
The successful candidate will work primarily 8 hour day shifts, Monday to Friday.
