Position Summary Through the lens of the Social Determinants of Health, the Health Promotion Supervisor leads, develops, implements, and evaluates health promotion initiatives aligned with the organization’s community health priorities.
The Health Promotion Supervisor helps to build inclusive and healthy communities by addressing barriers to health equity and fostering engagement in priority populations. This leadership role focuses on capacity building, education, and advocacy to empower individuals and communities to take control of their health.
The Health Promotion Supervisor collaborates with primary care, community partners and resident leaders to support initiatives that promote well-being, prevent illness, and enhance access to resources. This position requires strong interpersonal, facilitation, and advocacy skills to drive meaningful community change.
Job Specific Responsibilities Heath Promotion and Education This position builds capacity in health promotion and prevention activities through the development of health promotion programs. Using a lens of the Social Determinants of Health, this role develops, implements and evaluates health promotion initiatives at a community level. Grounded in connection with local community and informed by local population health data, the Health Promotion Supervisor will create new partnerships with Primary Care partners supporting initiatives that promote well-being, prevent illness and enhance access to resources.
Recognizing the challenges of our rural geography, this is a mobile role, with capacity to offer programming in local medical clinics, at WOCRC and in rural community settings including outside of the City (ie Arnprior, Almonte, Carleton Place). This position is being filled on a 12-month renewable contract basis
Health promotion and Community engagement
- Develops and implements community health promotion strategies that empower individuals and positive community health outcomes
- Engages priority populations to identify health and social issues, facilitating community-led solutions and promoting ownership.
- Engages and works collaboratively with neighbourhood medical clinics as a member of allied health teams in collaboration with physicians and other primary care partners to develop health promotion activities aligned with neighbourhood level community need
- Conducts needs assessments and environmental scans to guide program planning and strategy development.
- Acts as a resource for community members, providing connections to relevant information, agencies, programs, and services.
- Supports the formation and sustainability of resident-led and staff-supported community groups.
Program Supervision
- Provide direct supervision and leadership of the Health Promotion team of 3-5 community health focused positions.
- Develop and co-facilitate community development and health promotion initiatives informed by neighbourhood level community health data and consultation with health partners.
- Write and develop proposals for health promotion projects and programming
- Participate and facilitate local and regional /groups/committees/coalitions whose mandates relate to the identified community health issues.
- Develop strategies that contribute to strengthening individuals, partner organizations and communities; through, capacity building, and facilitation;
- Creates opportunities for knowledge exchange, best practices sharing, and service improvement among primary care partners.
- Identifies and facilitates the development of programs and/or services for members of the community addressing the identified need
- Coordination of health promotion strategies across the organization and in community.
- Assists with the overall functioning of the Centre and may participate in cross-team Management meetings.
- Collaborate/communicates with Managers and Senior leadership on Centre-wide issues such as service delivery, administrative supports, proposals for new services, development of policies, procedures.
- Participates on committees, action teams and working groups as required.
Service Coordination And Referral Support
- Ensures team is knowledgeable in local programs and services to coordinate referral and system navigation service delivery of staff team
- Coordinates access to learning opportunities, such as city-wide training sessions, community conversations, and wraparound service for team and primary care partners.
- Works closely with WOCRC staff teams to develop integrated support strategies for community members.
Evaluation And Monitoring
- Designs and implements evaluation frameworks to assess program effectiveness and community impact.
- Monitors project outcomes in alignment with funder requirements and organizational priorities.
- Stays informed on emerging trends in health promotion and community development to inform program improvements.
Partnership Development And Collaboration
- Strengthens partnerships with Primary Care, Ontario Heath team and Community Health and Resource Centre partners and other service networks to enhance service delivery.
- Leads city-wide initiatives to improve holistic and coordinated community care.
- Participates in broad-based community planning efforts to enhance local health and social support systems.
Qualifications Perform other related duties as assigned Education And Experience
- Masters or Graduate degree in health or social sciences (psychology, social work, criminology) or an equivalent combination of education and significant related experience.
- Three to five years of experience in community development/health promotion, crisis intervention, and/ or social work
- Excellent knowledge of community development values, principles and political processes.
- Experience in program and partnership development.
- Experience in evaluation and analysis of population health data
- Experience and demonstrated capacity for developing relationships and working with rural communities, multicultural communities, low-income communities, and high-risk families
- Demonstrated experience in-group facilitation and partnership development
- Experience working with high-risk families and knowledge of related community resources
- Excellent organizational and communication skills

- Other languages reflective of the community we serve are strongly preferred
Other
- This position requires a vehicle for travel between rural community locations.
- Valid driver’s license and access to a vehicle required, including $2 million in liability insurance.
- Valid First Aid and CPR (level C) required
- Availability to work occasional evenings and weekends when required
