Position Description

Unit Profile
The Reactivation Care Centre (RCC) is a specialized short‑stay program that supports patients who no longer require acute medical hospital care but need focused rehabilitation before returning home or transitioning to another care setting. Our interdisciplinary team delivers goal‑directed assessment, treatment, and functional reactivation to prevent deconditioning and ensure safe, effective transitions.
Grounded in unstoppable compassion, the RCC team works collaboratively to help patients with diverse needs achieve optimal wellness in a purpose‑designed environment that promotes mobility, engagement, and recovery. The unit plays a key role in enhancing patient outcomes, improving system flow, and supporting a dignified, person‑centred care experience.

Position Summary
The Nurse Practitioner functions as a member of an interprofessional team and within a shared care clinician model in the provision of specialty care services for the assigned patient population using advanced nursing knowledge, skill, clinical reasoning, problem solving and critical thinking.
The Nurse Practitioner works in the following domains within the Post Acute Care Portfolio: Clinical Practice, Consultation, Education, Research and Leadership/Administration. The Nurse Practitioner is authorized to perform extended health services which include comprehensive assessment, diagnosis, intervention, referral, management, and evaluation of patient care. All duties and responsibilities of the Nurse Practitioners follow the guidelines developed by the Registered Nurses of Ontario, the College of Nurses of Ontario, and St. Joseph's Healthcare.

Qualifications

  • Graduate of a Nurse Practitioner Program
  • Masters Degree in Nursing
  • Nurse Practitioner Registration in the Registered Nurse - Extended Class (NP- Primary Care, NP Acute Care) with the College of Nurses of Ontario
  • Faculty appointment or eligibility to pursue faculty appointment at McMaster University School of Nursing
  • BCLS certified
  • Member of RNAO preferred
  • Member in the following associations are an asset: Canadian Association of Rehabilitation Nurses, Nurse Practitioner Association of Ontario, and Gerontological Nursing Association of Ontario
  • Recent related clinical experience of 3 years
  • Expert level practice skill; advanced practice experience preferred
  • Recent research or evaluation experience
  • Recent leadership experience
  • Demonstrated commitment to the values of Service, Responsible Stewardship, Community Responsiveness and Respect
  • Excellent communication skills (verbal and written)
  • Demonstrated leadership, team building and interprofessional team skills
  • Computer literate for clinical applications and Microsoft Office
  • Excellent organizational, interpersonal, problem solving and decision making skills
  • Research, teaching and consulting skills
  • Knowledge of statistics and ability to analyze fiscal and patient care data
  • Ability to learn and apply hospital policy and procedures, relative legislation, collective agreements, Canadian Hospital Accreditation Standards, Professional Disciplines Standards for Ontario.
  • Solid understanding of rehabilitative and chronic/complex disease management models and processes
  • Recent adult education/program development experience

Responsibilities

  • Completes comprehensive and advanced health history and physical assessment
  • Identifies and orders diagnostic tests and procedures according to approved medical directives within the scope of practice
  • Collects, interprets and analyzes data from multiple sources and uses this data to formulate medical diagnoses within the scope of practice
  • Develops, implements and documents the medical plan of care for the patient
  • Works collaboratively with patients, internal medicine, physical medicine and family practice physicians and other members of the interprofessional team to develop the rehabilitative plan of care Facilitates a shared care model which includes the applicable services for the assigned patient populations
  • Prescribes pharmacological and non-pharmacological therapies according to approved medical directives
  • Evaluates and revises plans of care based on patient responses
  • Performs skills and invasive/non-invasive procedures as appropriate
  • Initiates interventions for the purpose of stabilizing patients in urgent and emergent situations, and to address daily medical needs
  • Completes necessary consultation and documentation during the course of the patient's admission, including but limited to formatted documents (core data elements, components of the medication reconciliation process)
  • Chairs interprofessional patient care rounds and documents the outcomes of such rounds
  • Demonstrates advanced knowledge and synthesis of advanced nursing practice for patients requiring rehabilitative or complex care
  • Assesses complex practice issues, and develops, implements and evaluates innovative approaches and programs addressing these issue
  • Provides consultation to nursing staff, managers, co-ordinators, physicians and other team members regarding clinical and professional practice issues related to rehabilitative and complex care
  • Acts as a liaison and consultant to other health care facilities/institutions regarding patient care and health services
  • Applies relevant research and a broad range of theories to clinical practice using critical thinking and decision making
  • Plans, implements and evaluates needs-based continuing education for staff and clinical partners to promote quality patient care for specific populations
  • Participates in patient and family education related to the clinical specialty and enhancing outcomes
  • Fulfills academic responsibilities to university as faculty member to assigned university classes, and in participating in program and course evaluations and revisions
  • Evaluates nursing and interprofessional research and assesses its impact on patient care and nursing services
  • Interprets, communicates and disseminates research findings to support patient care and recommends policy changes based on research
  • Provides leadership in program development, implementation and evaluation
  • Acts as a mentor to nursing colleagues and others to improve and support nursing practice
  • Provides leadership in development, implementation and evaluation of quality management programs/initiatives as they relate to rehabilitation and complex care
