Clinical Team Lead - Continuing Care

Position Responsibilities:
- Provide direct care by assessing resident health needs – recording of vital signs, discusses and records chief health concerns, performs tests, administers injections and medications, dressing changes, immunizations, etc.
- Monitor residents with complex medical needs.
- Places phone calls to pharmacies to order prescriptions; sets up referral contacts.
- Documents patient care observations and activities according to professional standards and procedures.
- Maintains required records including specimen, maintenance, daily, weekly, and monthly logs.
- Obtains specimens (throat, urine, stool, wound, sputum, other body fluids, etc.), using proper technique and following policies and procedures for labeling.
- Responsible for regular and ongoing communication to team members to ensure they are well informed about current activities, resident status, changes in assignments and daily routines.
- Assisting with the coordination of students for training, responsibility for training, orientation, competency assessment with the Director of Care.
- Responsible for supervising and providing leadership to LPN and HCA teams while on shift to ensure AHS and company standards are met.
- Provides ongoing clinical education, coaching, and mentorship to Health Care Aides, LPNs, and other care staff to ensure best practices and high-quality resident care.
- Leads orientation and competency development for new staff, including training on policies, procedures, and resident‑centered care standards.
- Facilitates team huddles, debriefs, and case discussions to enhance interdisciplinary collaboration and problem‑solving.
- Partner with Director of Care to ensure effective employee relations, conflict resolution and action plans as required.
- Participation in weekly meetings including but not limited to Director of Care, Executive Director, Recreation Therapist, Care Team, Physicians and Pharmacy.
- Maintains a professional standard of discretion and confidentiality.
- Provides leadership or assistance as required in emergency situations.
- Partner with Director of Care to ensure all requirements are met, audits completed and effective and thorough follow up.
- Audit processes, suite cleaning, resident care, Risk Assessment etc.
- Responds to changes in resident condition, initiates appropriate interventions, and communicates with physicians and families as needed.
Qualifications and Education Requirements:
- Licensed Practical Nurse/Registered Nurse, experience in gerontology preferred
- Current membership in good standing with CLPNA/CRNA and CPR/BCLS for healthcare providers is required
- Clear police information check, with the vulnerable sector, dated within the last 6 months
- Demonstrated skills in leadership, communication, teamwork, concern resolution process, and customer service
- Strong organizational and computer skills, ability to problem solve through critical thinking and self-education, and demonstrated knowledge and commitment to person-centered care
- Awareness of accident and injury prevention, hazard identification and control; adherence to safe work practices and procedures is essential in this position
Other Bona-Fide Occupational Requirements
- Physically and mentally fit to perform the duties of the position (may be required to lift, move, push, or pull objects weighing up to 75 pounds)
- Ability to work on his/her feet for a complete shift while constantly bending, stretching, walking, lifting, or moving things
