- HOURS OF WORK:
- The stated hours of work are approximate as schedules are subject to change with the operational needs of the Health Centre. Changes made to hours of work will follow the terms of the applicable collective agreement, company policy and/or legislation.
- WHAT YOU’LL DO:
- C1 is an acute medical unit with a focus on the Reactivation and Restoration of older adult patients. The goal is to provide care interventions that promote a patient’s ability to reach their highest level of function. The unit will offer a comprehensive patient-centered care for older adults through a strong interprofessional collaborative care model. Patients will be cared for by a dedicated interprofessional team that have a passion for providing specialized care of the older adult. Caregivers will be recognized as an integral part of the care team and will be included in all aspects of care and daily activities and will be updated regularly be informed on progress and changes in treatment and conditions.
- The focus of care for the team will be to provide dedicated support to identified patients and their families through a short admission and transition back to the community. The team will be responsible for the necessary linkages and collaboration with hospital and community partners that ensure patients are supported for a successful discharge.
- The attributes of this Reactivation and Restoration unit goes beyond just providing a setting for patients to recover from their acute illness. The key attributes of the unit are as follows:
- Specialized Care - For patients and their families, it provides an environment that offers care that is tailored to the unique needs of the designated population.
- Excellence - It fosters confidence in patients and their families that they are getting the specialized care they need by a team that has knowledge, skills, and expertise.
- People-centered - Delivering care in a space focused on functional enhancement, and individualized patient-centered programming in a senior-friendly environment. Caregivers are identified, included, and supported as essential care partners and are respected as members of the patient's care team.
- System Responsiveness - The attributes extend beyond the patients and their families into the community at large which will have an acute care centre that will have the capacity to support their healthcare needs.

- · Excellent organizational skills.
- · Demonstrated ability to work independently and as part of an interdisciplinary team.
- · Demonstrated commitment to continuing education.
- · Demonstrated good attendance record and ability to maintain same.
- · Basic computer skills and knowledge.
- · Non-Violent Crisis Intervention Training would be an asset.
