About the role

The Mount Sinai Academic Family Health Team – Vaughan site is looking for a Nurse Practitioner to join our interprofessional primary care team. As an integral member of the team, you will provide safe, high-quality, patient-centred care to a diverse patient population and work collaboratively with the interprofessional team to support comprehensive healthcare delivery within an academic family health setting.

In this role, you will provide comprehensive care across the lifespan, managing both acute and chronic health conditions while promoting wellness, prevention, and health education. You will also play a key role in supporting education and professional development across the team, including formal preceptorship of Nurse Practitioner students and supporting the clinical learning of other learners, such as residents and clerks. Through engagement in quality improvement, policy and practice development, and academic activities, you will help advance excellence in primary care practice. About The Mount Sinai Academic Family Health Team The Mount Sinai Academic Family Health Team delivers comprehensive, patient-centered primary care across the lifespan. Our goal is to support patients in managing their health and improving overall quality of life through integrated primary care and specialized programs.

The Family Health Team operates at two locations, the Granovsky Gluskin Family Medicine Centre in Toronto and the Sherman Health and Wellness Centre in Vaughan. The primary location for this role will be at the Vaughan site. Family Health Teams bring together multiple primary care providers to function as a coordinated care team. This model allows patients to access a broad range of services in one setting. The interprofessional team includes physicians, nurses, social workers, pharmacists, and other specialized health care providers. As a teaching clinic, the MSAFHT works closely with the University of Toronto Department of Family and Community Medicine, supporting medical students and residents. The team values education, research, quality improvement, and collaborative patient care.

In This Role You Will

  • Provide comprehensive, evidence-based primary care across the lifespan, including assessment, diagnosis, management, and follow-up of acute and chronic conditions
  • Promote health, wellness, and disease prevention through patient education and individualized care planning
  • Order, perform, and interpret diagnostic tests, and prescribe or adjust medications within NP scope of practice
  • Collaborate with physicians and interprofessional team members to deliver coordinated, patient-centred care
  • Contribute to clinical program development, quality improvement initiatives, and practice innovation
  • Support education and mentorship, including precepting NP students and guiding residents, clerks, and other learners
  • Develop, review, and implement policies, procedures, and clinical directives to support high-quality care
  • Support research, knowledge translation, and academic activities to advance practice
  • Promote culturally safe, equitable, and inclusive care, considering social determinants of health
  • Maintain accurate and timely documentation and perform other duties consistent with NP responsibilities in an academic, team-based setting

Job Requirements Mandatory

  • Completion of a Master’s degree in Nursing or equivalent NP program
  • Nurse Practitioner (Primary Health Care) Certificate or equivalent
  • Current registration (Extended Class) and in good standing with the College of Nurses of Ontario (CNO)

Preferred

  • Experience or eligibility for academic appointment
  • Membership in RNAO and other relevant professional organizations
  • Previous experience as an advanced practice nurse, educator, or in leadership roles

Skills and Knowledge

  • Proven expertise in the Nurse Practitioner role within primary care, including assessment, diagnosis, and management across the lifespan
  • Knowledge and experience in preventive care, chronic disease management, and population health principles
  • Strong clinical judgment, decision-making, problem-solving, and critical thinking skills
  • Ability to work independently while collaborating effectively within an interprofessional team
  • Excellent interpersonal, communication, and patient engagement skills, including working with patients and families in vulnerable situations
  • Demonstrated ability to support knowledge transfer, mentorship, and education of students and other health professionals
  • Ability to assist in the development, implementation, and review of clinical processes, procedures, and directives to ensure high-quality, efficient patient care
  • Demonstrated commitment to evidence-informed practice, continuous professional growth, and ongoing learning
  • Strong organization and time management skills
  • Proficient in electronic medical records and other digital tools supporting patient care
