DESCRIPTION:

Within this role the employee is accountable for contributing to the delivery of the Kingston Health Sciences Centre strategy. As an employee, one must demonstrate an awareness of and be responsible for actively promoting and supporting patient and family centered engagement and care in all we do.

Under the direction of the Pharmacy Manager, Operations and Systems and the supervision of the Pharmacy Supervisor, Drug Distribution, or under the direction of the Pharmacy Manager, Oncology Practice and Sterile Compounding, as assigned, and within the scope of practice of a Registered Pharmacy Technician defined by the Ontario College of Pharmacists, the Pharmacy Technician assists in carrying out the activities of the Hospital Pharmacy in accordance with the standards of practice, morals and ethics of the Pharmacy profession and the Vision, Mission and Values of KHSC.

The Pharmacy Technician works to the full scope of practice as described within the National Association of Pharmacy Regulatory Authorities (NAPRA) Model Standards of Practice for Canadian Pharmacy Technicians to support the Pharmacists and ensure the safe and accurate distribution of pharmaceuticals in the Hospital.

The Pharmacy Technician is accountable to the Ontario College of Pharmacists and to KHSC for his/her professional service to patients of the Hospital.

PRINCIPLE RESPONSIBILITIES & DUTIES:

  • Drug Distribution (core duties)

  • Prepare new and refill individual patient drug orders and prescriptions (counting, pouring, weighing, measuring, mixing, repackaging, labelling) in accordance with Hospital policy and Pharmacy Services standard operating procedures

  • Receive, process, enter and check drug orders, prepare labels and dispense interim medications for inpatient orders, leave of absence prescriptions and outpatient orders

  • Package and repackage solid and liquid oral dose formats and topical preparations

  • Compound non-sterile preparations including suspensions, creams and ointments

  • Comply with biennial recertification in accordance with the NAPRA Model Standards for Pharmacy Compounding of Non-Sterile Preparations

  • Participate in the appropriate and timely delivery of Pharmacy products

  • Monitor and maintain medications within Pharmacy stock, automated dispensing cabinets, floor stock cupboards and carts, and emergency response carts

  • Collaborate with the Pharmacist to confirm that therapeutic review of patient orders and prescriptions has occurred including notification of drug interactions, duplications and maximum doses alerts

  • Refer requests outside of scope of practice to the Pharmacist or Pharmacy Managers (Operations and Systems or Oncology Practice and Sterile Compounding)

  • Complete final check, sign off and release of prepared Pharmacy products (including new orders and prescriptions, reorders, refill prescriptions, floor stock medications, self - medications, leave of absence medications, non-sterile compounds, sterile compounds, repackaged medications, and chemicals) as determined by Pharmacy policy and standard operating procedures

  • Document according to hospital and Pharmacy standard operating procedures

  • Perform clerical functions associated with the Pharmacy department including answering telephone calls from other health care professionals, filing orders and completing records

  • Maintain drug supplies in good dating

  • Maintain storage requirements for all drugs including cold chain

  • Maintain quality control of equipment used in Pharmacy

  • Maintain locations where drugs are stored in good state of cleanliness

  • Comply with workload measurement records

  • Serve on departmental and related committees as required

  • Participate in continuous quality improvement activities in the Pharmacy department

  • Proactively identify and mitigate risk wherever it may rise to enhance patient safety

  • Understand and comply with safe work practices, processes, procedures and policies to prevent errors and enhance patient safety

  • Identify and report hazards, near misses and adverse drug events

  • Provide technical guidance, training and supervision to Pharmacy Assistants

  • Provide technical guidance, training and supervision to Pharmacy Technician students

  • Provide technical guidance and training to new Pharmacy staff

  • Maintain knowledge of drugs including additions to the Drug Formulary

  • Complete other technical duties within the legislated scope of practice of a Registered Pharmacy Technician

  • Work in compliance with the Health & Safety Act and its regulations in performing duties in a safe manner and follows all health and safety policies, procedures and legislation including, wearing the appropriate Personal Protective Equipment (PPE), as required

  • Perform other related duties as assigned

  • Floor Stock replenishment (as assigned)

  • Generate fill lists, prepare stock refills, and check for product accuracy

  • Restock automated dispensing cabinets, medication refrigerators, carts and cupboards

  • Monitor floor stock medications in patient care areas Rotate stock and remove expired products

  • Sterile Product Preparation (as assigned)

  • Perform sterile non-hazardous preparations compounding in accordance with departmental standard operating procedures Comply with annual recertification in accordance with the NAPRA Model Standards for Pharmacy Compounding of Non-Hazardous Sterile Preparations

  • Hazardous Preparations Compounding (as assigned)

  • Perform sterile and non-sterile hazardous preparations compounding in accordance with departmental standard operating procedures

  • Comply with annual recertification in accordance with the NAPRA Model Standards for Pharmacy Compounding of Hazardous Sterile Preparations Participate in on-call Pharmacy technician rotation and return to the Hospital when requested by the on-call Pharmacist

  • Controlled Drugs (as assigned)

  • Maintain pharmacy inventory and records for all locations stocking controlled drugs (KGH site Dispensary, HDH site Dispensary, OR Pharmacy) in accordance with legislation and Hospital policy and standard operating procedures

  • Organize the distribution of narcotics, controlled drugs and targeted substances to patient care areas according to Hospital policy and standard operating procedures

  • Coordinate destruction of controlled drugs Participate in audits and tracers as required

  • Inventory Management (as assigned)

  • Perform inventory and purchasing functions including ordering and receiving drugs from vendors, perpetual inventory management, expired product returns, submission of group purchasing organization product differential claims, monthly and annual physical inventory count and financial reporting

  • Order and distribute drugs for all KHSC Pharmacy locations and Renal satellites Support backorder management and maintenance of purchasing information in the Pharmacy Information System

  • Investigational Drug Service (as assigned)

  • Order, store and dispense investigational products according to Standard Operating Procedures (SOP) for Clinical Research set forth by the Network of Networks (N2)

  • Follow Good Clinical Practices (GCP) guidelines and meet all training requirements as set forth by institutional policies, study sponsors and/or Health Canada regulations

  • Perform billings of Principal Investigators for clinical drug trials support services

  • Support physicians with obtaining approval for drugs available through the Special Access Programme and coordinate ordering, receipt, storage, dispensing and billing Receive, store and distribute compassionate medications intended for use in ambulatory patients

  • Medication Reconciliation (as assigned)

  • Participate in admission medication reconciliation; compile the Best Possible Medication History (BPMH) through patient or family interview and review of appropriate patient medication history information sources, and document the home medication list in the patient’s electronic patient record

  • Identify and refer patients who require assessments, clinical analysis or application of therapeutic knowledge to the pharmacist

  • Identify and refer patients who have discrepancies between their current drug therapy and their recent or home drug therapy to the pharmacist

  • Document significant outcomes and discrepancies in the electronic pharmacy record according to standard operating procedures

BASIC QUALIFICATIONS:

  • Completion of a Pharmacy Technician Program curriculum meeting the competencies and outcomes set forth by the Canadian Pharmacy Technician Educators Association (CPTEA), the National Association of Pharmacy Regulatory Authorities (NAPRA), and the Canadian Council for Accreditation of Pharmacy Programs (CCAPP)

  • Current unrestricted registration with the Ontario College of Pharmacists as a Registered Pharmacy Technician (R.Ph.T.)

  • Previous hospital experience with unit dose and/or automated drug distribution system and intravenous admixture service (preferred)

  • Work experience in a community or hospital pharmacy practice for a minimum of 4 months within the past two years

  • Demonstrated commitment to the Mission, Vision, Values, Ethics and strategic directions of KHSC

  • Demonstrated ability to perform duties independently, reliably and effectively, with a high degree of motivation and minimal supervision

  • Demonstrated troubleshooting skills and ability to problem solve issues that may arise in carrying out the duties of this position
