Are you the one we’re looking for?

We are seeking a dynamic, highly motivated Seniors Outreach Worker who demonstrates our values of compassion, accountability, responsiveness and empowerment; to our clients, families and each other.

The Seniors Outreach Worker works within the Client Support Service Program by supporting older Newcomer Government Assisted Refugee adults 55+ reduce social isolation and connect them to support Services in the Community. Client support services operates in Hamilton and Brantford.

This position is a unionized, full-time, temporary position. This is a contract position ending in December 31, 2026. The rate of pay is $23.22 per hour.

Key Responsibilities:

  • Go on client home visits and community accompaniments when needed.

  • Provide community referrals to places such as Dentists, Doctors, ODSP, Mobility clinics and more.

  • Provide interpretation support as needed

  • Maintain up-to-date client case notes and individual client files.

  • Connect seniors with their communities through creating effective pathways to make sustainable connections with services and activities that will support them in remaining in their homes

  • Attend regular supervision and CSS team meetings 1x a month.

  • Participate in internal/ external workshops/training that is relevant.

  • Foster an environment that encourages and supports community participation and engagement

  • Build relationships with local service providers, social service agencies, community health/recreation centres, etc. who offer free or inexpensive educational and recreational activities

  • Attend and participate in community meetings focused on engaging vulnerable seniors

  • Other duties as assigned.

What you will bring:

  • Preference will be given to individuals with post-secondary education in the field of gerontology or a social service worker certificate.

  • At least two years’ experience in social services, working with seniors

  • Experience working with vulnerable seniors and newcomers.

  • Speaking a secondary language in Arabic, or Somali is an asset.

  • Excellent record-keeping skills, including case notes and documentation.

  • Strong understanding of support services in the community, specifically those related to seniors

  • Strong interpersonal skills including active listening, building rapport with clients, etc.

  • Good written and oral communication skills and strong computer literacy

  • Ability to work independently

  • First Aid/CPR and NVCI training is required

  • Driver’s license an asset.
