What you will do

The Clinical Supervisor will oversee and assist in the supervision of individual care for an assigned group of individuals receiving services through Specialized Community Services, to ensure high-quality outcomes and a best-practices approach to their care. The scope of service oversight for the clinical services will be defined by the Managers or Regional Leader. This will include assisting in thorough intake assessments, as requested by Operations or Clinical teams to ensure that our facilities and capabilities are in alignment with individuals’ needs, developing comprehensive service plans, as well as monitoring and adjusting individual care on an ongoing basis.

Individual Clinical Management

  • Assists in case management and coordinating clinical aspects of care for an individual as delegated by clinical or operational teams
  • Supports, as needed, a comprehensive individual intake assessment, risk assessment (including environmental safety) and care plan (individual support plan, plan of care) with achievable individual-centered goals
  • Develops strong working relationships with individuals within the caseload to establish trust and understand individual needs and preferences utilizing appropriate tools and templates
  • When required, liaise with stakeholders involved in an individual’s care to ensure coordination (e.g. psychiatry appointments, doctor/ dental / optometry appointments, pharmacist communication, receiving medical equipment).
  • Participates in review of support plans/goals regularly to ensure the plan is current and brings concerns to managers for support in resolution
  • Monitors adherence to the recreation plan as created by the Team Leader/ Activity Team with individual/family (as applicable) and is regularly reviewed with individual to ensure it meets individual need/preferences
  • On call where appropriate for clinical escalation
  • Participates in case conference calls/meeting with the funder to discuss clinical aspects related to individual that the Clinical Supervisor is supporting, and the attainment of individual goals established/delegated
  • Adheres to local clinical standards and process with all services, continuity of care provision and quality and safe individual services are in place, in accordance with local accreditation requirements
  • Directs customer service inquiries to the appropriate personnel (e.g. manager)
  • Assists in the regular monitoring of individual care and clinical documentation.
  • Reviews clinical documentation and care delivery to ensure standards are being met, including form completion, tracking sheets and risk assessments, and escalates any concerns to managers right away
  • Compiles monthly individual clinical data as required and assists managers with summary reports as needed
  • Supports managers in completing risk assessment and safety plan as requested
  • Supports completely of Critical Incident Reporting related to clinical concerns under direction of managers

What you will need to be successful

  • Education: BCBA, LPN, RPN, RN, RPN, or other allied health professional degree must be in progress
  • Standard First Aid and CPR level C
  • Clear Criminal Record Check with the Vulnerable Sector Search
  • Unrestricted Driver's License
  • Access to a Vehicle that you will be using for work
  • Minimum 1 year of experience supporting individuals with disabilities and complex service needs
