RAI COORDINATOR Position Type: Temporary Full-Time

Contract: 18 months

Shift Type: Day, no weekends ( subject to change) Hourly Pay Rate: $37.56 - $38.16

Bi-Weekly Hours: 75 Hours, 8 hrs/shift

Apotex–JHA is a 472-bed Long Term Care Home providing a holistic approach to caring for older persons requiring 24-hour nursing care, to enable them to continue to live their lives to the fullest and fulfill their goals and daily activities.

Responsibilities include but are not limited to:

  • Manages the coordination, completion and submission of the RAI MDS/InterRAI LTCF program
  • Ensures all RAI MDS 2.0/InterRAI LTCF documentation is completed by the deadline and reports any conflicts to the Director of Care and Resident Experience or designate
  • Ensures the RAI MDS 2.0/InterRAI LTCF program aligns with required practices, legislative requirements and organizational policies.
  • Contributes to the development of RAI MDS/InterRAI LTCF policies as needed
  • Works in collaboration with the Apotex Leadership and Interprofessional teams and other appropriate stakeholders and partners, to ensure data integrity and appropriate utilization and interpretation of the data
  • Acts as the RAI MDS 2.0/ InterRAI LTCF resource for all participating team members in the home, which includes but is not limited to training and annual recertification of users
  • Collaborates in ensuring the information derived from the RAI MDS/InterRAI LTCF assessments are used in the formulation of individualized resident care plans tailored to meet residents’ unique goals and needs.
  • Acts as the point person for team members in supporting them to complete annual recertification and maintains records
  • Seeks professional development through attendance at seminars, workshops, in-service programs, reading literature pertinent to the profession and other skills development initiatives

Qualifications include but are not limited to:

  • Successful completion of a recognized Practical Nurse program
  • Current certificate of competence in good standing from the College of Nurses of Ontario.
  • Current and acceptable Vulnerable Sector Screen from the police services in the municipality of residence.
  • Current Basic Cardiac Life Support (‘BCLS’), Level C.
  • Recognized First Aid certificate.
  • Current RAI MDS AIS Certification
  • Must possess a minimum of 3 years clinical experience
  • Must possess a minimum of 2 years RAI MDS 2.0 related knowledge and practical experience
  • Previous experience delivering adult education or facilitating adult education
  • Excellent verbal, written and listening communications skills
  • Ability to document concisely and appropriately
  • Demonstrated ability to effectively communicate and develop partnerships with clients and families
  • Good understanding of the relevant provisions of current legislation and regulations governing Long-Term Care Homes in Ontario
