About the role

We are looking for multiple Social Service Workers for London and Surrounding areas.

The Thames Valley Family Health Team is one of the largest family health teams in Ontario. With more than 125 physicians across 21 sites within London/Middlesex, Oxford and Elgin counties, the Thames Valley Family Health Team is committed to providing comprehensive collaborative primary health care to over 165,000 patients. The Family Health Team staff includes nurse practitioners, registered nurses, social workers, pharmacists, dietitians, occupational therapists, and other health care professionals who will work with physicians as an interdisciplinary team, each using their experience and skills to provide patient centred care. The Social Service Worker will assist patients in dealing with personal and social problems by delivering counselling, community services and social support programs. The Social Service Worker participates in the assessment, treatment and evaluation of individual, interpersonal and societal problems using social service work. Working in collaboration with an Inter-professional Healthcare Provider (IHP) team.

Requirements:

  • Current certification with the Ontario College of Social Workers and Social Service Workers
  • Diploma in Social Service Work from an accredited college.
  • Minimum 2-3 years experience as a social service worker in a primary care setting preferred.
  • Experience with walk-in counselling in primary care (benefit but not required)
  • Must be available to work evening hours
  • Proficient in Rapid Assessment models and working with individuals and families.
  • Ability to meet site-specific requirements.
  • Participate in program and resource development in collaboration with our team
  • Must be knowledgeable of the relevant legislation that will pertain to a variety of populations.
  • Ability to provide needs assessment, identification of goals that promote wellness, empowerment, and autonomy

Skills/Abilities:

  • Experience related to the delivery of efficient primary care, including the ability to share information.
  • Knowledge of relevant health care legislation and community mental health & addiction resources
  • Knowledge/experience with case management
  • Knowledge of financial, housing & transportation resources
  • Experience co-facilitating skills-based groups
  • Understanding of social determinants of health and their impact on health and well-being
  • Strong leadership, organization, time management and interpersonal skills
  • Demonstrated ability to work independently and collaboratively as a member of a multidisciplinary team.
  • Desire and ability to update knowledge and skills through various means including technology-based opportunities, courses, workshops, and conferences.
  • Experience in the use of an EMR, Microsoft Word, Excel, and Outlook
  • Demonstrated ability to be open and non-judgmental.
  • Effective interpersonal and communication skills (written/verbal)
  • Must have reliable transportation to allow you to travel to other locations to complete your work if needed

Salary Range: $51,432 - $62,722
