Position Summary
Reporting to the Director, Emergency, Mental Health, and Patient Flow, the Patient Flow Coordinator's primary purpose is to ensure RMH continually advances a culture of effective patient flow through the application of LEAN principles, consistent adherence to the Home First philosophy, ensuring the right patient is placed in the right bed, and supporting optimal bed resource utilization across all units. This includes mitigating barriers to patient flow.
Working collaboratively with internal teams and external partners, the Coordinator leads initiatives to improve flow, reduce ALC days, support Home First strategies, and advance organizational goals aligned with Ontario Health flow targets.
Key Responsibilities

Monitor, analyze, and report on patient flow metrics.

Collaborate with clinical teams to address discharge barriers and optimize bed utilization.

Department/Unit/Program Description
Ross Memorial Hospital is committed to delivering exceptional care and ensuring efficient patient flow across the organization. The Patient Flow team plays a vital role in supporting timely admissions, transfers, and discharges to enable safe, effective care delivery.
Essential Qualifications
- Active registration in good standing with a relevant professional college (e.g. CNO)
- Baccalaureate degree in a health discipline (Nursing preferred)
- Minimum five (5) years of hospital experience
- Strong clinical knowledge and experience
- Excellent interpersonal, communication, coaching, mentoring, and team building skills
- Strong understanding of systems thinking, systems analysis, value analysis, and continuous improvement principles.
- Advanced analytical, problem solving, and critical thinking skills
- Proven ability to facilitate change and support teams in complex and stressful situations
- Demonstrated commitment to patient and customer centred care
- Effective decision making and organizational abilities
- Commitment to professional development
- Strong listening skills and ability to help teams resolve issues respectfully
- Proven attendance record
- Comprehensive knowledge of social determinants of health and their impact on discharge planning, LOS, patient flow, and system capacity.
- Experience participating in and leading quality improvement initiatives (e.g., PDSA cycles, LEAN projects).
- Compliance with patient and staff safety policies and procedures
- Knowledge of and adherence to all regulatory and accreditation standards
- Demonstrated alignment with Ross Memorial Hospital values
- Must be able to fulfill physical requirements of the job, as per current Physical Demands Analysis.
Preferred Qualifications
- Master's Degree in related health discipline
- LEAN training/certification
- Change management experience
Hours of Work
Must be available 37.5 hours per week, Monday to Friday.
