About the position:
Reporting to the Program Manager, the In-STED CMHC will be responsible for supporting clients experiencing mental health challenges and addictions in the Dufferin community. Their main goal is to try and reduce the need for clients to have multiple visits at emergency departments by providing clinical assessments, treatment plans, system advocacy, and coordinated care in system transitions.
Duties and Responsibilities:
- Provide time limited, short term case management for individuals identified by the Headwaters Health Care Centre Crisis and Emergency team and/or SHIP Central Intake
- Review referrals and conduct clinical assessments to determine service needs
- Formulate a client-centered service plan and coordinate timely implementation
- Provide education on mental illness and crisis management
- Link to services to address individuals’ most critical needs
- Develop follow-up plans prior to transitions
- Connect clients to longer term service, if required
- Refer to partnering organizations and facilitate linkages to community supports
- Provide streamlined transitions with “warm transfers”
- Partner with clients and their families as members of the health care team
- Liaise with community supports and partners (i.e. mental health and addiction service providers, and family members) as required
- Work independently and collaboratively with a multidisciplinary team to carry out mental health recovery and support
- Understand and be able to integrate a philosophy of client-centred care into daily responsibilities
- Demonstrate a strong commitment to quality care and service delivery based on SHIP’s Mission and Values
- Ensure appropriate records are kept and make sure that statistical information is maintained as per agency standards
- Adhere to agency Health & Safety Program and the Client Safety Framework in the promotion of client safety
- Perform additional duties as required/delegated by Management
- You may be required to administer naloxone in response to suspected opioid overdoses, and must complete appropriate training to recognize overdose symptoms, safely administer naloxone via nasal spray or injection, and follow emergency protocols including contacting emergency services. You will receive training within the first eight weeks of employment
Qualifications:
- Post-secondary education in Social Work, Social Services, Health Services, or equivalent
- 3+ years experience supporting individuals with mental health challenges in the community
- Knowledge and experience implementing DBT informed approaches is an asset
- Knowledge and commitment to the values of psychosocial rehabilitation and recovery oriented practices
- Individual and group counselling skills
- Excellent assessment, risk management, and crisis intervention
- Knowledge of psychiatric symptomology and medication as well as suicide assessment and intervention
- Expertise in collaboration with healthcare services
- Knowledge of MS Suite (Word, Excel, Power Point and Outlook)
- A vehicle, valid driver’s license, and adequate insurance coverage
- Valid Certification in First Aid & CPR
- Training in ASIST and CPI
Reporting Function:
The CMHC will report directly to the Program Manager and Supervisor. He/ She will work with both internal and external partners and will interact with all levels within the organization. They will be a support system, resource, and an advocator for the team and Company.
Working Conditions and Physical Requirements:
- Hours are typically from Monday – Friday, 8:30am – 4:30pm but may vary depending on client needs
- This is a contract position until July 12th, 2027 to backfill an existing position
- Average amount of physical effort is required.
- Able to sit/stand for prolonged periods of time
- Able to answer and communicate telephone calls and general computer use
- Able to lift up to 20lbs (product, documents, etc.)
- Vehicle travel to sites for visits/walk-throughs and open doors as required.
