ABOUT THE POSITION:

As a member of the OATC health care team, the CARE Coordinator is responsible for supporting all aspects of the Call Center, email submissions from the CATC websites, and new patients intakes across our clinic network. The CARE Coordinator also supports CATC’s physician on-call program. The CARE Coordinator’s primary role is to engage and assist new patients in a timely and meaningful way. The CARE Coordinator will play an integral role in the patient experience and ensure that all new patients have a positive first experience as they take their first steps toward recovery.

WHAT YOU’LL NEED TO HAVE:

  • Completion of High School
  • Ability to establish therapeutic relationships with patients
  • Excellent verbal and written communication skills to liaise with diverse internal and external professionals
  • Excellent organizational, time management and prioritization skills.
  • Ability to work within a team environment and collaborate as an active participant with others in assisting in the delivery of care to patients.
  • Knowledge and/or experience working with diverse communities.
  • Ability to work within the policies, standards, procedures, vision, mission and core values of CATC
  • Sound knowledge of MS Office (Work, Excel, Outlook, Internet)

YOUR DUTIES AND RESPONSIBILITIES:

  • Collaborates with the health care teams in the development of appropriate treatment plans for patients of CATC
  • Answers calls from the Call Center
  • Triages email submissions and disperses or responds appropriately
  • Conducts telephone or PCVC intakes with new patients
  • Educates new or prospective patients on opioid use disorder, OAT, harm reduction,CATC services, locations, hours of operation, best practices, Hepatitis C, naloxone
  • Communicates effectively with clinic staff and MRPs to ensure continuity of care for new patients
  • Works within programs' quality assurance guidelines

Client Services:

  • Communicates with MRP, CCM and nurse around patient needs, change in status and all emergency situations**.**
  • Facilitates clients in OTN sessions as per protocol
  • Provides comprehensive program information to current and prospective patients of OATC
  • Responds to general inquiries and makes appropriate referrals within OATC health care team
  • Facilitates linkages to CATC’s Hep C Program, CATC’s Pharmacy services including naloxone and medications

Administration**:**

  • Verifies health card, populates and maintains patient profile
  • Books applicable intake appointments
  • Orients and trains new staff as directed
  • Provides support to other clinics with daily operations as required
  • Acts in accordance with the all relevant legislation, policies, standards, procedures and protocols
  • Other duties as assigned
