Posting Period: August 14, 2025 - August 20, 2025 The Assertive Community Treatment Team provides services 24-hours a day, 7-days a week, to persons with serious mental illness who have complex needs and very significant functional impairments. The services that the team provides are based on the assertive community treatment model which promotes community living, psychosocial rehabilitation and recovery. As a member of the inter-professional team, the Concurrent Disorders Specialist – Registered Nurse (RN) provides expertise in the area of addictions, assessments, interventions and programs. The Concurrent Disorders Specialist – RN employs an integrated treatment model in which mental health and substance use treatments are provided simultaneously. The Concurrent Disorders Specialist also contributes to the development of comprehensive client-centered assessments and treatment plans; and provides service coordination for an assigned group of clients that includes providing a range of treatment, rehabilitation and support services.

Skills & Qualifications Required

  • Bachelor’s Degree in Nursing; Master’s Degree preferred
  • Current and unrestricted registration with the College of Nurses of Ontario
  • Certification as an Addictions Specialist
  • Minimum three years of work experience providing services to adults with a serious and persistent mental illness, effectively accessing and employing community resources to meet the needs of the client
  • Experience working with clients with a concurrent disorder

Position Responsibilities

  • Acts as liaison and consults with community agencies and families to maintain coordination in the treatment process
  • Participates in daily organization of client related activities in collaboration with other team members
  • Provides ongoing assessment of clients’ mental illness symptoms and clients’ response to treatment to ensure immediate and appropriate interventions are provided in response to changes in mental status or behaviour, which put clients at risk (e.g. suicidal)
  • Provides education to clients related to medications, their potential side effects and interactions, and strategies for managing effects
  • Provides assessment of the severity of substance use and its impact on the individual using standardized assessment tools
  • Provides recommendations for the treatment of the substance use
  • Takes a lead role or participates in providing medication administration and medical services
  • Coordinates, schedules, and administers medical assessment of individuals’ physical health, suggesting appropriate referrals to community physicians for further assessment and treatment, and coordinates psychiatric treatment with medical treatment, in collaboration with the team psychiatrist
