Job Description Case Manager LPN Part- time Permanent (with potential to move Full-Time) Home Instead® is looking for a dynamic and caring person to join our team as a Case Manager LPN in Leduc for a permanent Monday to Wednesday position to start on a part-time basis, with the opportunity to move into a full-time role in the future . . An active LPN registration is required. The Case Manager LPN will work closely with families who are at the beginning of their care journey and are looking for care solutions for their loved ones, also supporting Care Givers, conducting training and holding family meetings.
Home Instead is a Provider of Premium Home Care services to our elderly clients. This is a very different nursing role, than working in a clinic or care home setting. Our LPN Client Care Coordinators will work in a far more comprehensive scope of care than working in a clinic or care centre. To Be Successful The Candidate Must
- Current LPN registration is required
- Have a drivers license with vehicle
- Be extremely articulate with Perfect Command of the English language.
- Have at least 2+ years of Home Care Experience
- Be able to work with a competitive customer group
- Be able to multi-task with a busy work pace
- Have confidence when communicating
- Sales experience would be helpful
This position involves speaking with family members who are searching for answers to their senior loved one’s care needs. This means you will interact with a variety of different people who are at different stages of their care journey, confident and strategic communication skills are required.
You will need to use influencing skills to explain to families why they should choose Home Instead for their loved ones. This role is responsible for effectively managing multiple client demands that come with their changing needs. This individual must be able to work in a fast-paced organization and have excellent communication skills, be service-oriented and have the ability to work well in a team environment. Your health care background will help you understand the unique challenges our clients face. You will be busy and we are looking for that special person who loves hard work! This is how we do Home Care! No 2 days will look alike as you will be visiting clients and their families throughout the city! (very different than a clinic or facility role) This is a permanent Monday to Wednesday position. The Case Manager LPN is expected to perform a variety of duties that relate to client care including care consultations with potential clients and family members, client/Care Giver introductions and quality assurance visits with existing clients. They use the consultative sales approach to determine each individual client’s needs to provide solutions and create a tailored service plan. They continuously evaluate the plan through a series of ongoing communications and visits to ensure high quality care, client satisfaction and retention as well as opportunities to increase service hours. The Client Care Coordinator will often speak with families who are unsure of which pathway is the “best” so our Coordinator must have a confident personality with the ability to communicate professionally.
This leadership position will include teaching, training our unregulated workforce. This will include classroom training, as well as in home visits to train and support our amazing Care Givers.
Qualifications
- Must have excellent written and verbal English communication skills
- Direct experience working with Seniors and their families
- 5+ years experience in healthcare
- Experience with sales and customer service
- Supervisory experience is an asset
