Program Overview

The Licensed Practical Nurse –Clinical Integration Nurse as part of the Medical Home Support Program provides comprehensive care to clients by assessing, identifying and facilitating various services in collaboration with other professionals within a multi-disciplinary team. The LPN – Clinical Integration Nurse accepts responsibility for continuing competency through ongoing professional development, including participation in education programs, research and continuous quality activities.

Position

The LPN- Clinical Integration Nurse will work in physician’s offices and participate in the provision of care to patients by applying nursing knowledge, skills and judgment to assess patient needs and provide nursing care in collaboration with Medical Home Support Program team members and community agencies or programs.

Reports to

Manager, Clinical Services

Education

  • Two (2) year post-secondary education in an approved Practical Nursing Program and an approved National Examination
  • Active registration and in good standing with College of Licensed Practical Nurses of Alberta (must be able to provide proof of registration)
  • Immunization Certification is required

Experience

  • Minimum of one (1) to two (2) years’ experience in a licensed practical nurse role; two (2) - three (3) years is preferred
  • Current BLS-C is required
  • Knowledge of chronic diseases and their management
  • Certification in Foot Care from an accredited body is an asset
  • Knowledge of how to navigate various EMR’s (MedAccess, HealthQuest, Ava)
  • Strong assessment skills

  • Ability to communicate in a clear and concise manner in addition to being able to identify the most appropriate method of communication
  • Comfortable and experienced working with marginalized and vulnerable populations
  • Ability to organize time, multi-task and set priorities to meet various deadlines and maintain a flexible schedule
  • Able to work with minimal supervision, organizational skills and ability to prioritize
  • Strong listening, problem solving and critical thinking skills

Monday – Friday (shifts & hours of work vary depending on the assigned Physician clinic. Evening shifts may be required).
