Overview The Registered Dietitian is an active member of a multidisciplinary team who is responsible for the delivery of patient-centered clinical nutritional counselling services. The services of the Registered Dietitian will serve to enhance the quality of primary health care services in the area and specifically with the Greater Peterborough Family Health Organization. This one-year contract is 15 hours/week and involves travelling to Apsley as well as within the City of Peterborough.
Responsibilities
- Identify nutrition problems
- Assess the nutritional status of patients
- Establish goal oriented treatment interventions
- Provide family/patient education and provide support to address nutritional needs
- Work collaboratively with the FHO based team to identify and respond to clinical priorities
- Develop care plans and monitor the effectiveness of dietary changes
- Provides individual treatment and group classes as needed
- Specialize in diabetes, cardiovascular diseases, gastrointestinal conditions, and malnutrition
- Provide support to adults and seniors, including those with special medical needs
- Maintains and organized appointment schedule
- Monitors and manages caseload
- Completes and submits workload measures data and time sheets as required
- Regular attendance to all FHT meetings is mandatory
Other related duties may be assigned in the interest of the efficient operations of Peterborough’s Family Health Team and the effective management of our patient populations.
Hours of Work: Monday to Friday, some occasional evening work may be required.
Qualifications
- Registered Dietitian with the College of Dietitians of Ontario
- Two years’ experience in a primary health care setting preferred
- Current Diabetes Educator certification an asset
- Excellent assessment, planning and organizational skills
- Knowledge of the principles of adult learning
- Demonstrated ability to facilitate both individual and group educational sessions using the principles of adult learning
- Experience with Telus PS Suites an asset
- Valid Driver’s license and access to a vehicle

- Excellent interpersonal skills and ability to work effectively with patients, staff, physicians and the community
- Demonstrated proficiency related to behaviour change counselling

Founded at the turn of the millennium, Peterborough Family Health Team (PFHT) supports the primary health care sector in Peterborough City and County. As one of the largest Family Health Teams in Ontario, the organization works with over 100 primary care physicians providing comprehensive care to their patients. Collectively, we have over 100 inter-professional healthcare providers, comprised of: mental health clinicians, nurse practitioners, pharmacists, a physician assistant, registered dietitians, registered nurses and registered practical nurses.
In addition to one-on-one care, PFHT administers many programs, services, and workshops as a means to redirect patients from the providers practice to a more focused area of care. This approach increases capacity at the practice level while providing patients with an opportunity to learn how to manage their health. These programs, workshops, and services are offered at no charge and derive from irrefutable patterns displayed in the perpetual symptoms patients present.
PFHT is a Queen's residency site, allowing our primary care physicians to have the chance to teach new physicians. As Peterborough has a robust medical community, the PGY1s and PGY2s have the opportunity to not only work in primary care but also in emergency care, palliative care, sports medicine, subacute care and so much more.
