About the role

Overview
The Direct Support Facilitator has the goal of helping build great lives for the individuals,
their families and the community we serve by fostering an enriching and responsive
community participation environment. Working as members of a larger connection of
supports, Direct Support Facilitators – Community Participation will collaborate with other staff members to implement meaningful development plans and programming/activity
schedules that flow from a client centered philosophy.

**Hours of Work :**Monday, Tuesday & Wednesday – 8am – 4pm
Qualifications

  • DSW, SSW or Social Sciences degree or diploma in a related field.

  • Two (2) years recent experience working with adults with a developmental disability preferred.

  • Strong commitment to the individuals supported by Community Living Hamilton

  • Positive attitude and a true appreciation for diverse abilities

  • A valid driver’s licence, clean driver’s abstract and access to a reliable vehicle to facilitate travel to various sites

  • Valid First Aid/CPR Certification

  • Valid NVCI Certification

Community Living Toronto has been a source of support for people with an intellectual disability and their families since 1948. We offer a wide range of services including respite, person-directed planning, employment supports, residential programs, and community-based activities. We are proud to support over 4,000 individuals and their families in 80+ locations across Toronto.

The “community living movement” began with families who wanted their children to live in the community, rather than institutions. Today, we continue to advocate for inclusive communities and support the rights and choices of people with an intellectual disability.
