About the role

This is a Temporary Full Time position for 12 Months

Sinai Health is looking for a Palliative Care Coach (Clinical) to work with community organizations to advance high quality and interprofessional palliative care delivery in Toronto region. In alignment with provincial direction established by Ontario Health and guidance developed by the Ontario Palliative Care Network, you will lead in championing practice change, with a focus on improving access to and quality of care, aligned to the Palliative Care Health Services Delivery Framework in the Community and the Ontario Palliative Care Competency Framework. Organizations that you will engage with may include long term care homes, primary care teams and other organizations or teams in the community serving residents/clients/patients with palliative care needs.

In this role you will:

  • Provide palliative care coaching and mentoring to staff and clinicians in participating community organizations, to build primary-level palliative care competencies
  • Collaborate with leaders of community organizations on local service integration, and change management initiatives, using quality improvement methodologies to achieve results
  • Consult on care delivery when the complexity of the patients’ needs is beyond the existing competencies of providers (in the community organizations)
  • Co-create the plans for regional implementation of the delivery framework in partnership with Ontario Health
  • Support implementation of a plan for measurement and reporting on progress towards change
  • Foster regional collaboration among health care teams and palliative care providers and contribute to broader system integration
  • Lead engagement and community-building activities with participating organizations
  • Champion efforts to ensure equity for patients and families with palliative care needs
  • Critically evaluate the quality of palliative care in participating community organizations using tools like organization self assessment
  • Build competency amongst front-line community providers in the delivery of primary-level palliative care through provision of education and mentoring
  • Guide palliative care quality improvement (QI) and practice change initiatives in participating community organizations
  • Assist with quality improvement initiatives to implement the Delivery Framework
  • Consult on clinical care as required, including as part of the palliative care team at Sinai Health
  • Perform other duties consistent with the job classification as required

Job Requirements

Job Requirements:

Mandatory

  • A degree in a registered health care discipline (e.g., RN, RSW, NP)
  • 5 years of experience in practice in palliative care

Preferred

  • A graduate degree i (e.g., MScN, MSW)
  • Experience working across health care settings (hospital. home, community)
  • Experience educating and or supervising health care professional staff
  • Additional training in gerontology and quality improvement experience
  • Program evaluation and/ or quality improvement experience

Skills and Knowledge

  • Knowledge of relevant legislation and reporting requirements, including the Fixing Long-Term Care Homes Act, the Excellent Care for All Act and the Compassionate Care Act and other pertinent legislation and regulations related to professional practice
  • Strong practice coaching and debriefing skills, with both individuals and teams
  • Strong consultation skills, and ability to use these with individual, team and community service providers
  • A commitment to developing equity-based, evidence-informed approaches for delivering palliative care services for all people in Ontario, regardless of their age or illness
  • A good understanding of regional priorities and local health care needs within Ontario’s health care landscap.
  • Ability to work well under pressure and use good judgment in assessing difficult situations
  • Ability to work independently and as part of a team
  • Aptitude for proactive problem solving using strong critical thinking and negotiating skills
  • Strong active listening and interpersonal communication skills for one-on-one coaching
  • Strong skills in designing education to effectively address palliative care training needs of interprofessional learners and applying in a culturally responsive manner
  • Supports the advancement of equity, inclusion, and diversity across Ontario's health care system.
  • Commitment to engage with First Nations, Indigenous, Métis, and urban Indigenous people as they self-determine their own approach to palliative care
  • Proficiency in educational technologies such as webinars and apps, as well as knowledge of Microsoft Outlook, Word, Teams, PowerPoint, and Excel
  • Access to a reliable and efficient mode of transportation to enable travel between community organizations

**Note:**A flexibllity will be required regarding hours worked. Travel within Toronto to community organizations (Long term care home, other) within the coach's assigned geographic catchment area is required
