Community Visiting Nurse (RN) - Evenings

Part Time: 22.5 hrs/week
We are currently seeking a compassionate and dependable Registered Nurse (RN) to join our team as a Community Visiting Nurse for evening shifts. In this role, you will deliver high-quality, patient-centered care in client's homes, helping them maintain their health, independence and dignity.
the Ideal candidate is a Licenced RN with strong clinical judgement, excellent communication skills, and a passion for community-based nursing. You must be comfortable working independently, managing your schedule, and adapting to diverse home care settings.
Acclaim Health announces the availability of one (1) Established Part Time Registered Nurse position to work Evenings starting at 4:00pm in all of Halton and HNHB Community including clinics. As an Established Part Time, the individual will be required to forty-five (45) hours in a bi-weekly period. This position requires the individual to work 1:3 weekends in addition to the statutory holiday rotation. There is also mandatory on-call rotation.
Pay per Visit Rate : $56.88 - $59.06
PRIMARY FUNCTIONS:
- Provide nursing care to individuals and families in the Halton and HNHB Community and Clinics.
- Organizes the day, sets priorities based on continuity of care, the type of care to be given and the client’s/family’s health needs.
- Develops, implements, evaluates, and modifies the plan of care outlining the strategies, orders and actions to achieve expected/desired outcomes.
- Assesses the health status of the client.
- Administers IV therapy including central line and peripheral line maintenance.
- Administers medications topically through natural body orifices, through tubes and by injections.
- Handles wound care, acute and chronic, for certain types and stages of wounds.
- Responsible for ensuring that the lap top is synchronized with the network at a minimum of twice a day, at the beginning and end of your shift, in order to receive information on and update client records.
- Attends team/POD meetings as required.
POSITION REQUIREMENTS:
- Minimum 1 to 2 years clinical practice in a community and clinic environments preferred.
- Current registration with the CNO.
- Current CPR certificate.
- Palliative experience required.
- Experience in wound care (complex and non-complex).
- Possess skills in negative pressure wound treatment, IV and pump, CVAD, catheter care and maintenance.
- Excellent interpersonal, communication and teaching skills
- Valid driver’s license and access to a vehicle
Must be legally eligible to work in Canada
