Position Summary Care Transitions Coordinator, Admin Coord / Patient Flow The Coordinator, Access and Flow plays a critical role in ensuring efficient and effective patient movement throughout the hospital and across sites. This position is responsible for coordinating all aspects of patient flow so that patients have timely and safe access to the most appropriate bed type that matches their clinical needs. Acting as an expert in patient placement, the coordinator identifies opportunities to maximize bed utilization and collaborates across multiple departments and multiple sites—including bed control, clinical units, portering, and environmental services—to maintain optimal flow. This role requires strong problem-solving skills, the ability to navigate challenging periods of high demand, and the capacity to resolve conflicts while maintaining a patient-centered approach. The coordinator supports NYG’s vision by ensuring patients receive safe, timely, and effective care, contributing to a positive patient experience. Collaboration with external sites and Ontario Health is essential for repatriation and interfacility transfers, ensuring compliance with guidelines and performance targets.
On a practical level, you will Patient Flow Management
- Oversee daily hospital capacity and patient flow by monitoring ED patients awaiting admission (EAA), isolation requirements, blocked beds, and temporary bed utilization to ensure timely access to care and efficient movement throughout the hospital.
- Collaborate with and support the Bed Control Specialist in coordinating patient placement and managing intra- and inter-unit transfers, utilizing NYG’s flow and access tools, standards, and protocols in partnership with multidisciplinary teams.
- Have knowledge of ALC best practices, ALC pressures and home first initiatives to ensure optimal and timely patient placement.
- Collaborates with others to identify opportunities to create capacity for admissions during periods of high demand for inpatient beds.
- Collaborate with clinical units, portering, and environmental services to maintain optimal patient flow, resolve barriers as they arise, and proactively anticipate and address potential challenges before they impact operations.
- Lead daily patient placement meetings with clinical and non-clinical teams to review patient flow metrics and hospital status, address discharge and flow barriers, and develop plans to manage anticipated challenges.
- Monitor morgue capacity and collaborate with unit staff, portering, funeral homes, and other stakeholders to ensure adequate space; escalate issues related to unclaimed bodies to the Manager, Care Transitions. Data and Reporting
- Verify, update, and submit compliance reports for ALC patients and associated orders from the General site and RCC using the Wait Time Information System (WTIS).
- Maintain accurate and up-to-date patient records by creating, viewing, and updating data. Manage waitlist entries, generate comprehensive reports, and oversee Message Failure Management (MFM) processes.
- Ensure data integrity through proactive monitoring and timely corrections. Conduct trend analysis to identify issues and escalate concerns to the Manager, Care Transitions when necessary.
- Verify, track, and submit Daily Bed Census Reports to Ministry of Health. Repatriation and Transfers
- Coordinate all repatriation and interfacility transfer requests in alignment with Ontario Health guidelines, ensuring performance targets are met.
- Monitor and update the Provincial Hospital Resource System (PHRS) to manage and respond to incoming requests.
- Collaborate with physicians, Bed Control, and unit leadership to facilitate timely review and decision-making.
- Ensure physicians have access to all necessary information to make informed decisions regarding care needs and appropriateness for acceptance.
- Organize incoming transfers with external organizations to ensure timely, safe, and efficient transitions of care.
Qualifications
- Undergraduate degree in a health care discipline required.
- Current member and in good standing with a professional college required
- Minimum 3 years’ recent clinical experience in an acute care hospital setting required
- Ability to work independently and effectively to assess patients’ placement needs Proven leadership skills
- Strong facilitation and conflict resolution skills to help resolve challenging situations
- Ability to assess risk, and effectively utilize resources to support patient acuity
- Experience in patient flow coordination is considered an asset.
- Excellent Microsoft Office applications skills
- Bilingualism in French/English an asset.
