Job Description:
Specific Duties and Responsibilities:
General Responsibilities
- Communicates with residents, families, team members and health care professionals and/or other departments within the Nursing Home concerning residents’ needs. (on admission and ongoing)
- Maintains resident confidentiality and respects resident’s right to privacy.
- Participates in multidisciplinary team meetings and ensuring follow-up of recommendations.
- Supports resident’s choices and abilities.
Provision of Care Responsibilities
- Contributes to the resident’s care through the performance of selected routine tasks and procedures as determined by the stability of the resident’s condition and the predictability of the resident’s responses.
- Follows established, individualized plan of care for each resident.
- Demonstrates flexibility in organizing and providing care based on needs of all residents.
Assessment:
- Contributes to resident assessment through observations and data collection during resident care.
- Reports changes in the resident’s condition and collects additional data, (including specimen collection) as directed by RN or LPN in a timely manner.
- Collects and records resident information including, but not limited to weight, intake, output and may monitor routine vitals.
Planning:
- Participates in planning care to organize clinical workload and sets priorities for carrying out assigned work.
- Collaborates with other team members.
Implementation:
- Participates in carrying out resident care plan and all assigned work.
- Assists the resident with activities of daily living according to individual needs.
- Assists residents at mealtime including tray set-ups, feeding and supervision of residents; verifies individual diet guidelines are followed; monitors resident’s food, fiber and fluid intake; prepares and distributes resident snacks and extra fluids.
- Reinforce appropriate health related information to resident’s families and other staff/team members.
- Assists residents with mobilization, range of motion and exercises; transfers residents and positions residents using proper body mechanics and equipment, such as mechanical lifts, transfer boards, slider sheets and positioning wedges.
- Transports residents to and from in-house activities.
- Responds to residents call bells.
- Applies basic interventions within established guidelines/directions.
- Administers procedures and treatments as directed.
- Performs the following in collaboration with Licensed Practical Nurse or Registered Nurse:
- Applies simple dressings, creams and ointments
- Provides palliative care to the residents and support to the families
- Monitors, documents and reports changes in resident’s health, condition and/or behavior
- Evaluates and maintains resident safety needs
- Documents resident care and routine observations according to established guidelines
- Seeks guidance and direction when required in completion of daily assignment.
- Maintains a clean and tidy environment for resident such as cleaning tubs, bedpans, basins, urinals, and other resident equipment.
- Replenishes supplies such as incontinent products, laundry and linen carts.
- Assists residents with packing and unpacking of personal belongings upon admission and transfers or discharge.
- Demonstrates healthy/constructive/respectful communication to support positive team culture.
Evaluation:
- Contributes to the evaluation of care by observing and reporting changes in the resident’s condition to the RN and/or LPN promptly, on a daily basis, and at care conferences and multi-disciplinary reviews.
Operational Support
- Receives and relays messages.
- Moves and transfers resident when directed.
- Collect garbage and dispose of waste.
Organizational Responsibilities:
- Demonstrates knowledge of Lakeview Manor’s philosophy, mission, policies and procedures, and conducts work in a manner consistent with this.
- Participates in the LVM’s continuous quality improvement initiatives to improve resident, staff, department and facility performance outcomes.
- Utilizes the facility’s resources, such as supplies and equipment, economically and safely.
- Develops and maintains good interpersonal and communication skills and utilizes these skills in work with residents, families, volunteers, visitors and other staff.
- Assist in the orientation of new personnel.
- Participates in department meetings; attends in-service programs, staff and committee meetings.
- Supports student training/practicum as indicated.
Individual Responsibilities:

Maintains knowledge of, and applies, policies and procedures, such as the Fire and Disaster Plan, the Infection Prevention and Control (IPAC) & safe resident handling programs (ATRM), and any other site programs.

Practices all safety and infection control rules and regulations,

Reports need of repairs to supervisor, or informs Maintenance personnel, using the approved communication tools.

Has a thorough knowledge of, and is able to demonstrate, the safe use and care of supplies and assigned equipment required in the provision of resident care.

Provides a comfortable, safe and clean environment for residents, volunteers, visitors and fellow staff.

Adheres to WorkSafe NB Acts & Regulations in all duties.

Maintains professional and personal development through participation in continuous education programs, in-services and other initiatives.
Qualifications:
- High school graduation or equivalent, plus an additional course in Personal Support Worker or Health Care Aid Courses or equivalency.
- Physically and mentally capable of performing assigned duties.
- Good attendance record.
- Ability to work in team setting and take direction.
- Good communication and interpersonal skills.
- Able to follow verbal and written instructions in English.
- Able to provide Criminal Background Check screening and Vulnerable Sector Check that is satisfactory to The Salvation Army, in its sole discretion, is required.
Compensation:
The target hiring range for this position is $25.14 to $25.98 with a maximum of $26.81.
Placement in the salary range will be based on factors such as market conditions, internal equity, and candidate experience, skills, and qualifications relevant to the role.
