Position Description

Unit Profile
The Reactivation Care Centre (RCC) is a specialized short‑stay program that supports patients who no longer require acute medical hospital care but need focused rehabilitation before returning home or transitioning to another care setting. Our interdisciplinary team delivers goal‑directed assessment, treatment, and functional reactivation to prevent deconditioning and ensure safe, effective transitions.
Grounded in unstoppable compassion, the RCC team works collaboratively to help patients with diverse needs achieve optimal wellness in a purpose‑designed environment that promotes mobility, engagement, and recovery. The unit plays a key role in enhancing patient outcomes, improving system flow, and supporting a dignified, person‑centred care experience.

Position Summary
A Physiotherapist is a health care professional who attempts to prevent or alleviate functional problems arising from physical, psychological and/or social origins. Prevention and treatment of an impairment, disability or handicap is accomplished through the use of physical assessment, education and treatment which are based on ongoing clinical research. The goal of physiotherapy intervention is to promote quality of life, self-responsibility and independence.

Physiotherapists at SJHH play a major role in patient’s length of stay. As one of the few professionals who usually remains constant during a patient’s stay, PT’s promote early discharge planning, optimize patient mobility and function and ensure patients are safe to return home. The PT supports his/her profession by advocating for or contributing themselves to an environment where ongoing learning, evidence-based practice, quality improvement and research are practiced and supported.

Qualifications

  • Bachelor's Degree in Physiotherapy required; Master's Degree in Physiotherapy preferred
  • Current Registration in good standing with the College of Physiotherapists of Ontario required
  • Membership in the Canadian and Ontario Physiotherapy Associations preferred
  • Hospital experience preferred
  • ADP Authorizer preferred
  • Basic computer skills for word processing and access to information
  • Excellent organizational and time management skills
  • Proven ability to function well within an interdisciplinary team environment
  • Able to work under pressure, prioritize workload and meet deadlines
  • Excellent interpersonal and communication skills
  • Exceptional analytical thinking and creative problem-solving skills
  • Demonstrated knowledge, sensitivity and expertise in the care of seniors and their families
  • Support with the center's overall program development
  • Gentle Persuasive Approach training preferred
