Flexible Assertive Community Treatment Team (FACTT) is a client-centered, recovery-oriented Mental Health Service delivery model that facilitates community living, psychosocial rehabilitation and recovery for people with serious and persistent mental illnesses who have not traditionally benefited from traditional outpatient programs. The Social Worker on FACTT is part of an interdisciplinary treatment team. The team is located off-site and provides service to clients in the community.

Duties:

As a member of the FACTT Team you will:

  • Use best-practice and innovative approaches to provide client-centered services including assessment, treatment and psycho-social rehabilitation.
  • Regularly collaborate with clients to develop and update Recovery Plans of Care (including crisis plans).
  • Provide direct assistance and support to clients regarding substance use, housing, employment, relationships and leisure activities, activities of daily living, and transportation.
  • Provide effective intervention, such as motivational interviewing with clients regarding their substance use.
  • Work collaboratively, cooperatively and effectively within the FACTT Team and with community partner agencies including hospital inpatient units and emergency.
  • Demonstrate thorough knowledge of the theories and practices of a professional Social Worker as they relate to family and individual interventions.
  • Provide support to family and informal caregivers of FACTT clients as needed.
  • Provide crisis assessment and intervention to clients on a 24 hour basis (rotating on-call evenings and weekends).
  • Assume responsibility for client’s assessments, their treatment, rehabilitation and support of assigned clients within the context of the FACTT Team.
  • Participate in the design, implementation and evaluation of program services to meet the needs of the program.
  • Participate in daily FACTT Team meetings to review clients and organize daily assignments.
  • Complete and maintain all necessary FACTT Team documentation in the Electronic Health Record (EHR).
  • Documentation to be maintained in accordance with hospital and College policies and timelines.
  • Assume responsibility for client’s assessments, their treatment, rehabilitation and support of assigned clients within the context of the FACTT Team.
  • Advocate with community partners for services and opportunities for FACTT clients.
  • Be familiar with the ROHCG Strategic Plan and the Goals and Objectives of the Community Mental Health Program.
  • Keep current with hospital policies and procedures and applicable legislation (Mental Health Act, etc.).
  • Complete and maintain all requirements for registration with OCSWSSW.
  • Keep up to date regarding professional standards by reviewing current literature, attending education events, networking with colleagues, etc.
  • Facilitate lectures and information sessions to clients and the general public.
  • Participate actively in relevant Program, service, hospital and community committees including academic (education and research), advocacy and system development as required.
  • Participate actively in hospital Social Work meetings, committees, clinical consultation groups and quality assurance requirements.
  • Provide supervision to Social Work students.
  • Work in a manner that is in compliance with staff and patient/client safety practices, policies and procedures of The Royal.
  • Ensure a work environment that is conducive to The Royal’s Anti-Racism, Harassment and Discrimination Free Workplace Policy.
  • Perform other duties as assigned.

Qualifications:

  • Master’s degree in Social Work.
  • Current registration and member in good standing with the Ontario College of Social Workers & Social Service Workers.
  • Must have minimum of three (3) years of clinical Social Work experience working in a mental health and substance use setting.
  • Training and experience in crisis intervention, assessment, consultation and addictions in a community based setting.
  • Knowledge of MHA.
  • Excellent knowledge of community resources and referral processes.
  • Collaborative approach to working with community partners.
  • Strong conflict resolution abilities.
  • Excellent advocacy skills.
  • Proven presentation, training and group facilitation skills.
  • Demonstrated ability to work independently.
  • Effective team skills.
  • Able to work in a progressively automated environment.
  • Requirement to work regularly scheduled day, evening and weekend shifts and to be available for on-call duty.
  • Valid Driver’s license and access to a reliable vehicle.
  • English level A- is mandatory in oral expression, oral comprehension, reading comprehension and written expression. Bilingual (French/English) is an asset.

Salary Range:

44.67 to 57.90 per hour
