About the role

The Carefirst Family Health Team (FHT) is seeking Full-time permanent / Part-time permanent / Casual Physiotherapist to join our innovative, interdisciplinary team dedicated to delivering high-quality, community-based care.

About Us

Carefirst FHT is committed to delivering exceptional, comprehensive primary care through a collaborative, team-based model that includes family physicians, nurse practitioners, nurses, social workers, dieticians, physiotherapists, and medical receptionists. We have a robust Chronic Disease Management and Prevention program, including a Diabetes Education Program, and are actively involved in many of the Ontario Health Team’s primary care initiatives.

Our strong community partnerships ensure coordinated, high-quality, patient‑centred care across the continuum. We work closely with the Carefirst Seniors & Community Services Association to provide truly integrated services.

On‑site & Integrated Programs/Services include:

  • Ontario Health atHome (formerly Home and Community Care Support Services)
  • MINT Memory Clinic
  • Ontario Structured Psychotherapy (OSP) Program
  • Specialist clinics: psychiatry, cardiology, endocrinology, nephrology, optometry, geriatric medicine, gynecology, chiropody, and audiology

Clinic Locations

We currently operate two well-established clinics in Scarborough and Richmond Hill. Beginning in Fall 2025, we will be expanding with five new locations to support continued growth and community needs.

Current clinic locations:

  • 300 Silver Star Blvd, Scarborough (Midland / McNicoll)
  • 420 Hwy 7 East, Richmond Hill (West Beaver Creek / Hwy 7)

New clinic locations:

  • Within Richmond Hill / Thornhill
  • 360 Highway 7 East, Richmond Hill (Highway 7 / Chalmers Rd)
  • 105 Gibson Centre, Markham (Steeles Ave East/Warden Ave)
  • Unionville Commons, Markham (Unionville Main Street/Highway 7 East)

Employees will be assigned to a location that aligns with their preferences and residence, with occasional rotation to support clinic operations.

We are currently seeking a Physiotherapist to join our Family Health Team.

Terms: Full Time Permanent / Part Time Permanent / Casual

Responsibilities:

  • Provide community-based physiotherapy services within full scope of practices outlined by the College of Physiotherapists of Ontario, including but not limited to assessment, diagnosis and education intervention

  • Diagnose and assist diverse clients with acute and chronic health conditions and rehabilitation needs, from the lens of chronic disease prevention and management

  • Provide physiotherapy services independently and within a team, including rehabilitation assistants

  • Ensuring practice is consistent with professional standards and the ethical framework as outlined by the College of Physiotherapists of Ontario and organizational policy, practice and values

  • Demonstrating commitment to professional life long learning by participating in continuing education activities to meet the needs of the patient population served and the standards of the profession

  • Performing reflective practice, analyzing and evaluating physiotherapy knowledge and modifying practice accordingly to improve the quality of patient care

  • Performing cross-functional and other responsibilities, as assigned and/or requested

  • Maintain accurate and timely clinical documentation adhering to agency standards

  • Participate actively in program development and evaluation

Requirements:

  • Master of Science in Physiotherapy or equivalent
  • Registration and licensed to practice (in good standing) with the College of Physiotherapy of Ontario
  • 3 years related clinical experience in the community setting
  • Valid First Aid & CPR certification
  • Demonstrated capability to plan and recommend continued self-care programs
  • Strong interpersonal, organizational and problem-solving skills
  • Shown capacity to make independent judgment and work without supervision
  • Proficiency in English; a second language reflecting local community, such as Cantonese, is an asset
  • Valid driver’s license and access to personal vehicle for business purposes
  • Current vulnerable sector check (completed within one year)

Carefirst Seniors & Community Services Association is a non-profit community services agency. Since 1976, we have provided community support, home and personal health care services in a holistic manner to seniors, individuals with disabilities, and those with medical needs in the Greater Toronto, and York regions.

With a staff of 500 members and the help of a pool of 1,200 active volunteers, we serve over 7,000 clients a year, including seniors and the physically disabled; 1,500 of whom are "home-bound" and frail.
