Approximate Duration: 12 months

This position is a 12 month contract supporting the "Aging with Dignity Project", funded by the Homelessness Reduction Innovation Fund through the Canadian Alliance for Ending Homelessness (CAEH). In partnership with Homelessness Services, Niagara Region Mental Health, Seniors Services and in-kind supports other community organizations, the Aging with Dignity Project aims to provide a multi-disciplinary, collaborative approach to providing necessary supports and service connections to older adults (55+) who are newly experiencing homelessness due to a variety of reasons, with the overall goal of preventing them from becoming chronically homeless if they are willing to receive supports to become rehoused in an appropriate setting; and to provide appropriate transition supports, resources and connections to assist individuals who move into to permanent housing, supportive housing or Long Term Care to do so successfully and have decreased risk of returning to homelessness.

Additional work locations of this position include shelters, and street outreach including encampments.

This position currently falls within our hybrid model**, allowing the employee to** typically work a minimum of 50% of your time at your regular work location and the other 50% of time at home.

Job Description

JOB SUMMARY

Provide individual, couple, family, and group counselling services for persons with severe and persistent mental health concerns. Provide crisis stabilization, treatment, case management and case consultation services. Provide social work intervention and treatment and act as a professional resource to a diverse, interdisciplinary team. Provide screening and psychosocial mental health assessments to individuals referred to the program for the purpose of triage, determining service needs and eligibility status. Act as an external resource to other mental health and community service providers.

QUALIFICATIONS

EDUCATION

  • Masters degree in social work required
  • Must be registered as a member of the Ontario College of Social Workers and Social Service Workers (OCSWSSW) at the time of application

KNOWLEDGE/EXPERIENCE

  • Minimum of 3 years clinical social work experience in assessment and counselling in a mental health setting is required
  • Clinical experience in community mental health preferred
  • Formal training in evidence based best practice is preferred. Examples include: cognitive behavioural therapy, dialectical behaviour therapy, group psychotherapy, family therapy, trauma therapy, addictions and concurrent disorders treatment
  • Working knowledge of relevant provincial legislation (e.g., Mental Health Act, Child and Family Services Act) pertaining to services provided by a community agency
  • Knowledge of community resources relevant to the provision of mental health and related services
  • Formal training in mental health assessment and a working knowledge of mental health diagnosis are required

SKILLS

  • Must demonstrate excellent communication skills, both verbal and written
  • Computer literacy is required
  • French language skills are an asset

SPECIAL CONDITIONS

  • Must provide own transportation and be able to travel throughout the Niagara Region to other offices, work locations, or sites as authorized by the Corporation for business reasons. Employees who use their personal vehicles for approved Region business shall ensure they are adequately insured with their personal insurance carrier. It is the employee's responsibility to possess and maintain a valid driver's license of the appropriate class to carry out their job in accordance with Corporate Policy.
  • Support clients in their travel to locations in the community when it is relevant to the client’s recovery plan as per program policy (which allows the option of staff transporting clients in their personal vehicle)
  • In accordance with Corporate Criminal Records Check policy, the position requires the incumbent to undergo a Criminal Records check, including vulnerable sector screening, and submit a Canadian Police Clearance Certificate prior to commencing employment
  • Be available to work flexible hours including weekend and/or evening hours as required
  • Vaccinations as recommended in Public Health policy
