OVERVIEW

Lumeo, a Regional Health Information System (RHIS) consisting of six health partners in the Southeast Ontario region, supports the vision of “One standard of care and journey for the people we serve”. The regional system enables the sharing of information seamlessly across the health-partners, connecting the personal health information into a single cohesive story and reducing the variability of care provided.

The Primary Care Informatics Medical Lead will work closely with the Lumeo operational leadership team to provide guidance and direction on behalf of Primary Care Providers (PCPs) across the Hastings Prince Edward (HPE), Lennox & Grenville (LLG), and Frontenac, Lennox & Addington (FLA) Ontario Health Team regions. This role will act as a key liaison between community-based physicians, the Southeast region’s OHTs, and Lumeo's Operational Leadership, Executive Forum and Advisory groups to ensure the region's digital health strategy enhances patient care, streamlines provider workflows, and strengthens the connection between primary and hospital-based services.

Reporting to the Regional Vice President, Digital Health Systems, the Primary Care Informatics Medical Lead will drive meaningful change and shape the future of integrated care in southeastern Ontario. The Primary Care Informatics Medical Lead is a standing participant in relevant Lumeo governance, advisory, and escalation forums, representing primary care perspectives in regional decision-making. This important role ensures Primary Care perspectives are meaningfully embedded in regional digital decisions, workflows, and change initiatives.

RESPONSIBILITIES:

Clinical Leadership & Strategy:

  • Serve as the chief Primary Care clinical subject matter expert for Lumeo.
  • Work closely with Regional Chief Medical Information officer and Chief Interperprofessional Practice Officer and operations team to support the development and execution of a regional strategy for utilization, standardization, and optimization particularly in regard to inclusion and impacts on PCPs.

Primary Care Engagement & Advocacy:

  • Build and maintain strong relationships with family physicians, nurse practitioners, OHTs, community physicians, Lumeo Operations team, and Lumeo Partner organizations to foster a collaborative approach to digital health. Serve as the primary advocate for PCPs within Lumeo’s governance and advisory forums.
  • Serve as a liaison between clinical teams outlined above, IT experts, and executive leadership, facilitating effective collaboration among groups with different priorities.
  • Ensures communication and engagement to primary care and OHTs to ensure the following outcomes:
    • Transparency of operational activities and decisions
    • Awareness of high-risk workflows being discussed and mitigation strategies
    • Organizational readiness for change

Workflow Design & System Optimization:

  • Understands Physician needs and build relationships with Physicians to assure widespread acceptance, adoption, and meaningful use of the clinical information system

  • Influence the co-design of Lumeo workflows to ensure they are intuitive, efficient, and supportive of seamless transitions in care. Champion system enhancements that reduce administrative burden and improve the clinical experience for PCPs.

  • Promotes an environment of engagement between physicians, staff, and leadership

Change Management & Issue Resolution:

  • Serve as the ultimate escalation point for complex, system-level issues affecting PCPs. Provide expert clinical guidance and coordinate with technical teams to ensure timely and effective resolutions.

  • Be a champion and advocate for RHIS adoption, communicates the Regional Lumeo strategic vision and act as the liaison between local physicians, regional team, and local information technology and informatics departments

  • Work closely with the Lumeo regional team on change management strategies.

CORE COMPETENCIES

  • Provincial Health System Knowledge: Experience collaborating with provincial digital health organizations such as Ontario Health Digital, OntarioMD, or the SGFP Digital Health committee.

  • Passion for Innovation: A forward-thinking mindset with knowledge of and experience with emerging digital health solutions, AI tools, clinical decision support (CDS) systems, and modern scheduling platforms.

  • Personal Effectiveness: characteristics that are typically associated with emotional intelligence and that focus on self-knowledge/awareness and awareness of how best to conduct oneself in relation to others

EXPERIENCE & QUALIFICATIONS

  • Licensed Family Physician or Nurse Practitioner currently practicing within one of the Lumeo partner hospitals

  • A practicing Primary Care Provider in the Hastings Prince Edward (HPE), Lennox & Grenville (LLG), or Frontenac, Lennox & Addington (FLA) Ontario Health Team (OHT) regions.

  • Minimum of 5 years of hands-on experience as a power user or lead for a major EMR system (e.g., Epic, Cerner, Telus Health, QHR). Advanced experience in workflow configuration, reporting, and interoperability with hospital systems (e.g., OLIS, HRM), sufficient to support issue escalation and clinical troubleshooting. Must have regular access to an active community EMR using OLIS and HRM.

  • Demonstrated experience in a leadership, mentorship, or project management role, with a proven ability to lead change and build consensus among peers.

  • Exceptional interpersonal, communication, and presentation skills, with the ability to translate complex technical concepts for clinical audiences.

  • Strong analytical and problem-solving skills, with an understanding of clinical data, quality improvement methodologies, and population health management.
