Functional Recovery Support Case Manager
About the role
Make a difference in a growing, innovative mental health organization and centre of excellence in people-centered care. Work with supportive colleagues who are passionate about improving lives and champions for quality and continuous learning.
As a Recovery Support Case Manager, you will manage a caseload of individuals engaged in treatment or post-assessment services, supporting them across all stages of recovery — from assessment and engagement through to sustained recovery and return to work (RTW). You’ll work as part of a collaborative team to deliver tailored recovery planning, connect clients to needed supports, and ensure progress is monitored and communicated effectively.
If you’re motivated by work that helps people achieve lasting recovery and meaningful return to work, and you thrive in a role that combines client service, problem-solving, and collaboration, you’ll love it here.
What You’ll Be Doing
- Manage a caseload of clients at the direction of a Clinical Lead, implementing and reviewing recovery plans
- Connect participants to community resources, including health care, substance use treatment, mental health services, and return-to-work supports tailored to their goals
- Collaborate with clients, employers, referral sources, and healthcare providers to support recovery and return-to-work outcomes
- Monitor compliance, attendance, and progress, escalating concerns to the Clinical Lead when needed
- Document case activity and maintain accurate, up-to-date clinical records and reports
- Prepare progress reports, manage data and billing submissions, and participate in case and team meetings
- Support departmental projects, provide coverage for colleagues as needed, and contribute to ad-hoc initiatives
What We’re Looking For
- Bachelor’s degree in Psychology, Social Work, or related health discipline preferred (equivalent experience considered)
- 2–5 years of experience in administration or case management, with preference for case management background
- Knowledge of case management principles and practices is an asset
- Ability to read and interpret policy wording
- Strong organizational, decision-making, and communication skills
- Ability to work independently while contributing effectively to a team
- Strong problem-solving, conflict management, and attention to detail
- Caring, approachable, objective, and committed to exceptional client service
- Proficiency with MS Office (Word, Excel, PowerPoint)
