DESCRIPTION:
The Case Manager provides services in a manner consistent with HDH Mission, Vision and Values and that reflects the core values of Patient and Family Centred Care (dignity, respect, information sharing, participation, and collaboration); practices in accordance with the standards of professional practice and ethical values as defined by the College of Occupational Therapists of Ontario; works as a member of a multidisciplinary team in the Heads Up! Early Intervention in Psychosis Program, a regional specialized outpatient mental health service that responds to the early intervention needs of 14 to 34 year old clients in Southeastern Ontario; uses advanced skills in assessment, treatment and psycho-education to deliver innovative client centred services to individuals and families who are experiencing a first episode of psychosis; works in a program that uses an interdisciplinary approach to provide clinical services to individuals, couples, families and groups in Belleville, Napanee, Kingston, Brockville and Smith Falls; provides skilled services at an advanced level of practice and works in close collaboration and partnership with other mental health and addiction providers in the Southeast region; utilizes an effective scope of clinical practice for direct service, consultation, program development, teaching and evaluation; includes collaborative interdisciplinary strategies in their professional practice that promotes effective problem solving; demonstrates expertise in public speaking and participates in the planning, presenting and evaluating of psycho-educational information and programs; collaborates and communicates effectively with clients, families and all members of the health care team of multidisciplinary professionals and support personnel; participates in service and hospital wide activities and programs that model and promote a safe and healthy patient-centred environment and that contribute to quality improvement and risk management initiatives.
QUALIFICATIONS:

Current Ontario registration in good standing as a regulated health professional (Social Worker, Occupational Therapist or Psychotherapist)

BCLS certification at the Health Care Provider level and current First Aid
-
3-5 years clinical work experience in a mental health setting working with a broad range of patient groups including youth and adults.

Direct experience working within an Early Psychosis Intervention Program Preferred
-
Must have an automobile, a valid driver’s licence and be able to travel in the Southeastern Ontario Region on a regular basis
-
In-depth knowledge of Ontario Mental Health Act, Health Care Consent Act, Powers of Attorney Act, Substitute Decisions Act and other relevant legislation
-
Demonstrate knowledge, skill and ability in dealing effectively with patients with severe mental illness.

Extensive knowledge of evidence based practice and current experience in the assessment and treatment of complex mental health disorders
-
Demonstrated knowledge of DSM-V diagnostic criteria for psychiatric disorders most prevalent with youth and adults; specific working knowledge of Psychosis an asset.

Proven good attendance

Proven effective interpersonal and communication skills

Demonstrated ability to effectively conduct a needs assessment, develop collaborative care plans, educational tools and programs and facilitate individuals learning experiences for patients

Demonstrated ability to effectively deal with conflict situations and proactively and effectively resolve issues impacting on quality and continuity of care

Excellent time management, organizational and prioritization skills

Proficiency with Microsoft Office applications including Outlook, Word, Excel

Capable of coping in a physically and mentally demanding work environment

Ability to function independently as well as interdependently

Demonstrated ability to work as part of a multidisciplinary team of psychiatrists, allied health and nursing professionals and external agency representatives

Proven ability to make critical decisions independently and rapidly

Training in CBT, dialectical behavior therapy (DBT) and solution-focused brief therapy is preferred
-
Knowledge of assessment and treatment for substance use, addiction and concurrent disorders is an asset.

Bilingualism (French and English) preferred

Current Crisis Prevention Institute (CPI) Nonviolent Crisis Intervention training preferred
-
Proven ability to attend work regularly.

Satisfactory criminal reference check and vulnerable sector search required.
