What you’ll be doing: Reporting to the Manager, Clinical Services, the Clinical Care Partner - LTC Operations will support multiple communities within a designated region. This role focuses on supporting clinical operations, standardizing care practices and driving compliance with regulatory and organizational standards. Compliance & Quality Improvement

  • Develop, monitor, and follow up on compliance plans with care community teams.
  • Conduct quality assurance audits to assess care and service effectiveness and guide improvements.
  • Collaborate with the DOC and SSO Quality & Health Information Team to interpret and act on RAI-MDS quality indicator outcomes.
  • Identify risks and support care teams in implementing mitigation strategies to ensure a safe environment for residents.

Process and Practice Development

  • Support the creation and implementation of evidence-based processes and practices.
  • Lead care community and organization-wide improvement initiatives in collaboration with the Regional VP Operations and local teams.
  • Provide subject-matter expertise for the development and review of policies and procedures.

Coaching, Education, and Leadership

  • Coach and mentor care community team members to enhance care, quality and operational practices.
  • Lead collaborative clinical teams (e.g., falls, skin & wound, responsive behaviours) using a train-the-trainer approach.
  • Facilitate education sessions and orientation programs for new initiatives and nursing leadership development.
  • Recommend training and professional development opportunities to strengthen team capacity.

Operational and Strategic Support

  • Review monthly business performance and identify quality successes and opportunities for improvement.
  • Participate in strategic planning and support staffing model implementation with the DOC and nursing scheduler.
  • Prepare and distribute documented site visit reports.
  • Support the Regional VP Operations as part of the regional leadership team to drive consistent quality outcomes.
  • Participate on internal and external committees as required.

Collaboration and Communication

  • Foster positive relationships with internal and external stakeholders.
  • Facilitate change through collaboration, shared problem-solving, and alignment with organizational goals.
  • Promote knowledge sharing and communication across care communities.

Health and Safety

  • Adhere to the organization’s Health and Safety Policies and Procedures and comply with the Occupational Health and Safety Act.
  • Perform other related duties as assigned.

Education & Experience:

  • Registered as a Nurse (RN) in good standing with the provincial regulatory college Registered Nurse required.
  • Certificate of Gerontological Nursing (Canadian Nurses Assoc.) preferred.
  • Post Baccalaureate education in Nursing, Health Sciences or related field is preferred.
  • Knowledge and understanding of provincial Long Term Care regulatory requirements and inspection process.
  • 3 years related management and relevant experience in long-term care.
