In this community nursing role, as a Registered Practical Nurse (RPN) you will have scheduled visits to care for patients with palliative care needs as part of a palliative care team comprising of Nurses, PSW’s and Allied Health members. You will work with patients of all ages with differing complexity of care and play a key role in helping patients navigate through their care journey.
ParaMed is also pleased to partner with HealthForceOntario (HFO) as an approved employer for the Community Commitment Program for Nurses (CCPN) which provides a $25,000 retention bonus to eligible nurses in exchange for a 24-month, full-time commitment.

What you’ll do:
- Using appropriate assessment tools/techniques to assess patients’ individualized needs
- Executing and continuously monitor care plans
- Engaging patients using a collaborative approach by building rapport and trust
- Maintaining and update professional patient records related to patient encounters and plans
- Liaise with other members of the client’s palliative care team
What you bring:
- Registration with the CNO as a Registered Practical Nurse in good standing
- Palliative care and PPS trained: Palliative Care Fundamentals / CAPCE and LEAP Core Training (or will be trained within 3 months upon hire) preferred.
- Valid driver’s license and access to a vehicle is required
- Willingness to travel to and from patient residences within a designated area
- Passion for making a difference in your community
- Required on-call coverage, including after hours to ensure patient safety and to meet patient care needs – one instance every 8 weeks
