Position Information This position is for an outpatient unit.
Rotation/Shifts Days, 8 hour shifts.

Basic Requirements
- Master's in Social Work with a clinical concentration from a university of recognized standing;
- Member in good standing of the Ontario College of Social Workers and Social Service Workers;
- Experience working with Cardiac patients;
- Excellent organizational, problem solving and decision making skills;
- Experience/knowledge with patients applying for financial assistance such as CPP, ODSP, LTD and EI Sickness;
- Assisting patients with Instrumental concerns such as housing, medication coverage;
- Knowledge of a variety of theoretical frameworks and treatment modalities such as crisis intervention, systems theory, etc;
- Knowledge of the Health Care Delivery System;
- Knowledge of legislation, acts and regulations such as Ontario Works, Public Hospital and Mental Health Acts;
- Ability to provide individual counseling;
- Excellent knowledge of discharge planning, community resources and the placement process;
- Excellent assessment and crisis intervention skills;
- Demonstrated skills in teaching and consultation;
- Superior written, verbal and presentation skills;
- Ability to multi-task;
- Superior interpersonal skills;
- Proficiency in English and French - oral expression (advanced level) and comprehension (advanced level).
Language proficiency requirements are based on the nature of the position and the level of interaction with French and English-speaking clients.
