Position Summary:
Bayshore is seeking a Registered Nurse to join our amazing team in the role of Care Coordinator. The RN Care Coordinator is responsible for coordination of care and service provision from receipt of referral to discharge. The Care Coordinator will focus clinical expertise with relationship-building skills to ensure high-quality care for clients while fostering and developing key partnerships that support business growth.
Quality and Continuous Improvement:
- Oversees the overall services provided to WSIB funded clients in Ontario
- Foster and develop key relationships with WSIB referral sources and Rehab hospitals
- Primary point of contact for WSIB Nurse Consultants; coordinating the delivery of care to ensure care is integrated within members of the home health care team and communicating as needed with other health care providers in the workers care team
Interdisciplinary Liaison:
- Ensure coordination of care from referral to discharge; including following up on nursing or rehabilitation recommendations with WSIB and other health care providers (ie. Family Physicians) as needed
- Provide clinical oversight to ensure the recommended care plans delivered by personal support workers and nurses are individualized and within best practice
- Provide clinical and quality oversight and guidance to Clinical Leads to ensure there is clear and objective information when a recommendation is made regarding a workers level of care
- Provide clinical oversight and support to Clinical Leads care providers delivering personal support work and nursing services
Education and Professional Development:

Participate in Case Conferences with the Care Team and WSIB as required

Case conference for discharge/transition planning as appropriate to ensure safe discharge to home; Care Coordinator will facilitate a coordinated discharge plan with the inpatient team inclusive of arranging any necessary equipment or supplies in collaboration with the WSIB

Attendance at meetings requested by customers

Prepares Consultative Nursing Evaluations, Care Re-authorizations Forms and specific insurance documentation as necessary

Participate in quality activities and continuous improvement initiatives in keeping with the company's Quality Management System.
Health and Safety:
- Maintains confidentiality of client and corporate information and discusses same only with appropriate Bayshore personnel.
- Participates in proactive Health & Safety activities while performing all duties. Is responsible for notifying immediate Supervisor of any Health & Safety risks or concerns. Responsible for completing Accident Reports for all direct reports who injure themselves on the job, within 24 hours of the incident.
Support Branch Operations:

Manage branch operations in the absence of senior management when needed.

Use electronic health records to improve clinical outcomes and practice standards.

Based on review of a referral and leveraging clinical expertise, engage the most appropriate care providers based on clinical skills/expertise required and ensure consistency of care provider team in order to enhance the quality of experience for the worker

Qualifications:
Education & Training:
- Registered Nurse with a minimum of 3 years experience and current registration with the provincial regulating body.
Experience:
- At least three years of nursing experience; experience in working in a supervisory capacity; demonstrated ability to handle all aspects of human resources; experience in business development preferred.

Proven computer technology skills e.g. Microsoft applications, and ability to use virtual meeting forums.
