Position: Senior Manager, Care Services (Full-time, Contract until June 2026)

Hours of Work: 35 hours per week

Reports to: Director of Care Services

Position Summary The Senior Manager, Care Services leads a team of Care Managers, strategically develops evidence-based recommendations for service development, and leads quality initiatives in preparation for an accreditation survey.

Responsibilities

  • Coordinates accreditation activities and serve as Accreditation Coordinator
  • Develops evidence and analysis for program development and clinical services, and work with the organization’s leaders to support planning
  • Leads infection prevention and control (IPAC) practices for the Care Services team and co-chair the IPAC committee.
  • Applies experience and education in Quality, Safety & Risk (QSR) initiatives and Co-Chair the QSR Committee
  • Supervises and mentors the Care Management and Addiction Support Program interprofessional team
  • Applies advanced knowledge in community healthcare to ensure adherence to internal policies, leading practices, ethical standards and consistently meet client safety responsibilities and high-quality care standards.
  • Fosters a collaborative, innovative and learning focused environment to support a diverse healthcare team deliver high quality integrated person-centred care in the community, optimizing performance across operations and clinical outcomes
  • Actively collaborate with external health system partners related to innovative services design ensuring seamless client care transitions
  • Monitors and reports on key performance indicators, initiating improvements or process changes as identified

Education :

  • Master’s degree
  • Membership in good standing with a regulatory body in Ontario e.g. Social Work, or Occupational Therapist

Experience & Skills :

  • Minimum seven (7) years of experience in clinical background, health administration, or public health
  • Three (3) years of recent experience with progressive responsibilities as a clinical manager, preferably in a community setting
  • Experience in working with a senior population, knowledge of specialized geriatric healthcare principles and community-based care and social services
  • Experience with program development, implementation and evaluation
  • Experience with budget management, data analysis, reporting, funding applications and evaluations
  • Solid knowledge of principles, practices and methods related to leading interprofessional teams
  • Understanding of health equity, the social determinants of health, harm reduction principles, trauma-informed care and anti-oppression
  • Sensitive to the cultural needs of clients from various ethnic groups
  • Excellent interpersonal, communication, decision-making skills
  • Strong computer and technical skills and experience with digital health systems

Other:

  • Vulnerable Sector screening required
  • Proof of full COVID-19 Vaccination or Medical Exemption Certificate required
  • Able to work at the site every day
  • Must be available to work occasionally afternoons and weekends.
  • Must be available to work in-person every day of the week.
  • Must be available to conduct occasional home visits.
