About the role

The Concurrent Disorders Assertive Community Treatment (ACT) program is a new innovative partnership initiative between Unity Health Toronto (St. Michael’s Site) and Toronto Public Health. The program relies on an inter-professional team to provide community-based assertive community treatment services (home visits, community visits) to individuals living with serious mental illness and substance use disorders in downtown Toronto.

The Occupational Therapist assists with treatment, recovery support and outreach services in collaboration with an interprofessional team. In person-centered services are provided in collaboration with other community agencies to enable individuals to harness their own personal strengths and abilities.

DUTIES & RESPONSIBILITIES:

  • Provision of Occupational Therapy to promote, maintain or restore health and well- being through engagement in meaningful occupations in the areas of self-care, productivity and leisure.
  • Occupational therapy services will include assessment, treatment, consultation, discharge planning and education to patients, family members/caregivers, external agencies and SMH staff
  • Utilizes assertive outreach strategies, to locate, engage and maintain clients in a helping relationship. Delivers service and supports in the client's environment of choice in the community (e.g. home or other community setting). Adjusts frequency and length of outreach visits based on client needs and goals and team consultations
  • Works effectively as a member of an interprofessional team, collaborating with other disciplines to provide comprehensive patient care
  • Contributes to the academic mandate of the organization as an educator, resource or support for staff, students and members of the academic and professional community
  • Identifies and addresses educational needs for patients and their families
  • Demonstrates commitment to ongoing professional development in order to continue to provide optimal patient care
  • Is committed to, and participates in, ongoing quality improvement in the organization
  • Provides patient care in accordance with all professional and organizational standards

QUALIFICATIONS:

  • Must have a Master's degree in Occupational Therapy

  • Must be currently licensed by and in good standing with the College of Occupational Therapists of Ontario for practice in all applicable clinical areas

  • Current CPR certification

  • Excellent communication, critical thinking and interpersonal skills

  • Proven flexibility and adaptability in a fast-paced environment

  • Excellent time management, organizational and prioritization skills as relates to patient care

  • Demonstrated commitment to and involvement with own profession (e.g. membership with professional association, etc.)

  • Knowledge of recovery as it applies to community mental health settings

  • Knowledge of case management models and functions

  • Knowledge of medical, pharmacological and other treatment strategies for individuals with serious mental illness and substance abuse disorders

  • Must be willing to provide rotational on-call, evenings and/or weekend availability as directed by the Program.
