Job Description

The Hemodialysis Social Worker is integral to the Social Work professional team and is part of a multi-disciplinary team reporting to the Patient Care Manager, Dialysis, with professional accountability to the Professional Leader, Social Work.

Accountabilities:

  • Responsible for the care of patients and families with ESRD

  • Participate in yearly strategic planning initiatives and corporate professional development

Qualifications

  • Current registration, and in good standing, with Ontario College of Social Workers and Social Service Workers

  • Ability to provide coverage in critical care and throughout the organization as required

  • Masters degree in social work required

  • Three to five years of experience working with renal patients

  • Knowledgeable in community resources (i.e. Kidney Foundation)

  • Membership in CANSW preferred

  • Knowledge and experience working with relevant legislation (i.e. SDM Act, POA, consent and capacity, etc.)

  • Knowledge of the psychosocial impact of chronic illness on the person and the family

  • Knowledge in Nephrocare preferred

  • Familiar with Ontario Renal Network guidelines preferred

  • Excellent communication and interpersonal skills

  • Ability to work as part of a multi-disciplinary team

  • Excellent organization skills and ability to set priorities as required

  • Good critical thinking and problem-solving skills required

  • Excellent attendance, punctuality and work record

  • Computer proficiency preferred

  • Must be able to travel to all Osler sites

  • Must demonstrate Osler's Values of Respect, Excellence, Service, Compassion, Innovation and Collaboration

Additional Information

Hours: Currently days/evenings, 8 hour shifts (subject to change in accordance with operational requirements)

Salary:
Minimum: $44.69 per hour
Maximum: $57.93 per hour
