HOURS OF WORK:
The stated hours of work are approximate as schedules are subject to change with the operational needs of the Health Centre. Changes made to hours of work will follow the terms of the applicable collective agreement, company policy and/or legislation.
Type of Shifts: All shifts - Days, Nights, Weekends & Holidays
WHAT YOU’LL DO:
The RPN, as one of the providers of health care, has an important role in promoting health, in preventing illness, and in helping clients attain and maintain the highest level of health possible. S/he is responsible for providing comprehensive care to patients and families, with more predictable outcomes, less complexity, and lower risk for negative outcomes who may or may not be clinically stable. The RPN practices at the top of their scope of practice in accordance with the College of Nurses of Ontario Professional Standards and within the 3-factor framework.
This position is a cross appointment between the Palliative Care and Complex Continuing Care Programs at PRHC. The RPN is assigned to care for inpatients in Palliative Care, Complex Continuing Care, or both. The RPN’s time will be scheduled on either C2 or D2 each shift and are expected to develop their knowledge, skills and experience in Palliative Care and Complex Continuing Care.
In Palliative Care the RPN provides pain and symptom management, activities of daily living assistance and end of life care to the patient and emotional and psychological support to the patient and their extended family.
In Complex Continuing Care the RPN individualizes care and optimizes health of adults with complex medical conditions and co-morbid conditions through evidence-informed interventions. The RPN uses a gerontological approach in their assessments and interventions with patients and families. The RPN uses best practice guidelines to direct their practice with geriatric patients.
WHAT YOU’LL BRING:
- Current Certificate of Competence from the College of Nurses of Ontario
- Current medical nursing experience, preferably in palliative care, in a hospital or hospice setting
- Current BCLS
- Demonstrated commitment to continuing education
WHO YOU ARE:
- Familiarity with specific diagnoses and pathologies associated with palliative care
- Excellent assessment skills
- Excellent verbal communication and written documentation skills
- Excellent interpersonal skills and ability to work well with patients, families, staff and physicians
- Excellent organizational skills
- Demonstrated ability to work independently and as part of an interdisciplinary team
- Computer skills and knowledge.
