About the role

Summary JOB SUMMARY:Within the context of a patient, client and family centred model of care and, in accordance with the Mission, Vision and Values, and strategic directions of Providence Health Care, promotes a safe, respectful, and civil working environment for patients, residents, families, visitors and staff. Reporting to the Patient Care Manager or designate, obtains and inputs patient information for the admission and/or discharge of outpatients at Holy Family Hospital Rehabilitation Services into computerized information system. Performs reception and clerical functions such as receiving and providing routine information to visitors/patients, answering and relaying telephone messages, typing documents, filing, and faxing information as directed or according to established policies and procedures. QUALIFICATIONS:Education, Training, and ExperienceGrade 12, medical terminology, one year’s related experience or an equivalent combination of education, training and experience.Skills and Abilities- Ability to communicate effectively both verbally and in writing.- Ability to keyboard at 40 wpm.- Ability to organize work.- Ability to deal with others effectively.- Ability to operate related equipment.- Physical ability to carry out the duties of the position. 1.Receives patient and families at desk. Obtains and verifies required personal information from patients for admission to Rehabilitation Services. Provides routine information to patient regarding appointments.2.Receives completed admission packages from Admission Coordinators or other designated staff. 3.Enters specific patient information such as demographic information, appointment times into computerized information system. Sends out confirmation of admissions.4.Processes outpatient discharges faxing them to primary caregivers, closing encounters in computerized information system. Checks to make sure discharge summaries are complete, if not follows up with staff to obtain. Disassembles charts and sends to health information management. 5.Types relevant documentation and forms from drafts or direction such as lists, schedules and agendas using word processing software and distributes or posts documentation to appropriate areas.6.Performs receptionist duties such as answering the telephone, taking and relaying messages, distributing mail, receiving visitors to the hospital, providing information and directing to the appropriate person and/or areas. 7.Telephones patients to provide or obtain routine information as per established procedures.8.Performs clerical duties such as booking meeting rooms, filing, typing, emailing and faxing documents, assembling and disassembling binders and charts, and typing and distributing minutes from meetings.9.Prints, posts, faxes, and files the approved staffing flow sheets. 10.Maintains filing systems for material, correspondence and general information. Ensures stocks of standard template forms, pamphlets are on hand.11.Performs other related duties as assigned.
