Temporary Full-Time Primary Care Practice Facilitator - Dryden Hub The Kiiwetinoong Healing Waters Ontario Health Team (KHWOHT) is dedicated to fostering collaborative partnerships and advancing integrated care solutions for our communities. We are seeking a highly motivated professional to join our team as Primary Care Practice Facilitator. In this key role, you will support clinics through their journey toward achieving deliverables in Ontario's Primary Care Action Plan (PCAP). Working closely with a physician champion and other KHWOHT change agents, you will catalyze clinic-level changes, encourage continuous improvement, and coach teams through processes that enhance patient care.

This position requires flexibility, including occasional evening and weekend hours, as well as frequent travel to member clinics within the Dryden hub and occasional travel to regional partner organizations in Sioux Lookout, Red Lake, and Thunder Bay.

Responsibilities Facilitates change management and process improvements while building capacity across primary care teams.

  • Develop professional relationships with clinic teams, physician leaders/champions, KHWOHT staff, and provincial peers/support programs.
  • Work with clinical and administrative staff to streamline referral processes across three primary care teams in Ignace, Dryden, and Vermilion Bay.
  • Broker relationships across clinics, KHWOHT, northwestern Ontario, and provincial programs to build awareness and coordinate efforts in advancing PCAP implementation.
  • Understand clinic team goals for improvement and connect their ideas to the PCAP.
  • Introduces provincial quality improvement opportunities, programs, and supports to clinic teams.
  • Work with physician champions to motivate and inspire clinic teams to act.
  • Use structured approaches to change (such as the Model for Improvement) to achieve clinic goals toward PCAP deliverable implementation.
  • Build quality improvement capacity within clinic teams and other primary care staff.
  • Champion and support the implementation of quality improvement and evaluation measures to sustain improvements.
  • Facilitate PCAP measurement and evaluation needs with clinic teams.
  • Support and encourage clinic teams to share their improvements and lessons learned with others within KHWOHT, across northwestern Ontario, and provincially.

Qualifications The ideal candidate will possess the following key competencies, record of experience and academic preparation:

  • Undergraduate degree or equivalent (education and experience considered).
  • 5 years' healthcare experience; 2 years in primary care (preferred).
  • Skilled in project management, quality improvement, and facilitation.
  • Proficiency with Ontario Electronic Medical Records (EMRs).
  • Advanced Microsoft Office Suite proficiency.
  • Valid driver's license and reliable vehicle.
  • Exceptional verbal and written communication skills.
  • Proven ability to engage stakeholders and build strong partnerships.
  • Expertise in coaching, mentoring, and change management.
  • Highly adaptable with strong priority management skills.
  • Strong analytical skills to drive evidence-based improvements.
  • Cultural competence with sensitivity to diverse populations.
  • Demonstrated initiative and organizational awareness.
  • Skilled in conflict resolution, negotiation, and diplomacy.

This is a Non-Union position; salary will be commensurate with qualifications and experience $39.96/hr-$43.32/hr., plus up to 21% in lieu of vacation and benefits.
