Position Overview

Working with out-patients and their families, the social worker provides direct social work service. The social worker functions as a member of the interdisciplinary team and provides psycho-social assessment and a range of interventions to assist patients and their families in dealing with personal, family or social difficulties that impact upon their level of wellness. The incumbent will act as an active and pro-active member of the health care team in the Pulmonary Rehabilitation Program, providing both urgent service as well as short term follow-up with identified patients and their families. The social worker is also a clinical resource to the Pulmonary Rehabilitation Program team members, the Misericordia Health Centre as a whole and to other social work professionals within the facility based on the training, experience and abilities demonstrated by the incumbent.

Experience

Minimum of two (2) years within the last five (5) years related clinical social work experience. Previous experience within a long-term care setting is an asset. A demonstrated ability to develop, prepare and present group educational programs is also required.

Education (Degree/Diploma/Certificate)

Bachelor’s degree in Social Work (BSW) from a Canadian Association for Social Work Education (CASWE) accredited school/faculty, or a Provincially authorized post-secondary program, or the approved equivalent if internationally educated. Cardiopulmonary Resuscitation (CPR) training requirements for this position shall be in accordance with the Employer policy.

Certification/Licensure/Registration

Must be registered with Manitoba College of Social Workers. May be required to hold a valid Manitoba driver’s license and have access to a reliable vehicle. Cardiopulmonary Resuscitation (CPR) training requirements for this position shall be in accordance with the Employer policy.

Qualifications And Skills

Works within MHC’s core values, philosophy, vision, CQI goals, policies and procedures. Ability to function as an effective member of an interdisciplinary team, including collaboration, team work and good communication skills. The incumbent must be able to work independently and communicate in a sensitive and effective manner with both consumers from the community as well as internal customers of the service. The social worker must demonstrate the ability to work in a positive manner with external agencies and their representatives. She/he must be able to present concise, meaningful written reports and articulate the social/psychological needs of the clients. The social work role demands a high-degree of professional judgement, personal initiative, creativity and adaptability to a constantly evolving health care environment. The incumbent must demonstrate a strong commitment to the profession of Social Work, operate within the parameters of generally accepted principles and standards of practice as well as adhering to the Canadian Association of Social Workers’ Code of Ethics.

Physical Requirements

May be exposed to infectious diseases, blood and body fluids, toxic materials, noise, allergens, physical and emotional stress. May be occasionally required to work extended hours. May be required to work a variety of shifts including but not limited to days, evenings, and weekends. The incumbent must be able to demonstrate flexibility and resourcefulness in dealing with changing priorities, crisis situations and multiple demands on their time. The social worker must be able to work with fluctuating levels of stress and as an effective team member with other disciplines.

This position requires a current satisfactory Criminal Records Check (including Vulnerable Sector Search), Child Abuse Registry Check and Adult Abuse Registry Check as conditions of employment. The successful candidate will be responsible for any service charges incurred. A security check is considered current if it was obtained no more than six (6) months prior to the start of employment.

The WRHA legal entity includes Churchill Health Centre, Deer Lodge Centre, Golden Door Geriatric Centre, Golden West Centennial Lodge, Grace Hospital, Middlechurch Home of Winnipeg, Pan Am Clinic, River Park Gardens, Victoria General Hospital, WRHA corporate programs, and WRHA community health services.
