position Purpose summary:

  1. Assesses patient/resident/family's actual and potential strengths and limitations including physiological, psychological, socio-cultural, and spiritual needs.

2.Provides a supportive environment for patient/resident/family. Promotes patient/resident/family involvement in health care decisions; provides information and opportunity to discuss concerns; respects personal choices.

  1. Contributes to the development and ongoing changes to the individualized plan of care, including attending and providing input into multidisciplinary care planning meetings and family conferences.

  2. Provides nursing care according to identified policies and procedures, technology and FHA standards and evaluates outcomes. Records observations and reports problems and/or changes to designated staff.

  3. Performs admissions, discharges, and transfers of patients/residents according to agency operating policies and procedures.

  4. Documents and updates all relevant care and procedure information to ensure patient/resident safety and comfort.

  5. Operates and maintains effective functioning equipment related to patient/resident care such as bathtubs, scales, and mechanical lifts.

  6. Assists in maintaining a safe and clean environment for patients/residents and staff.

  7. Assists in the orientation of new staff, students, and other members of the Health Care Team.

  8. Participates in continuous quality improvement and program evaluation activities, education sessions, team conferences, meetings, and committees.

  9. Performs other related duties as assigned.

Qualifications:

Education, Training and Experience –

  1. Graduate of a recognized practical nursing program or equivalent combination of education, training, and experience. Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM).

  2. One (1) year’s recent, related gerontological nursing experience, or an equivalent combination of education, training, and experience.

  3. Current Level 1 First Aid Certificate

Required Skills/Knowledge:

  1. Must be able to speak and write English competently, and to understand written and oral instructions.

  2. Ability to deal with others effectively.

  3. Ability to organize work and implement a plan of care.

  4. Knowledge and competencies to work at full scope of practice as currently required by the BCCNM, including administering medications.

  5. Ability to operate related equipment.

6.Physical ability to carry out the duties of the position.

Physical: Physical ability to carry out all the nursing-related duties.

PREFERRED SKILLS/capabilities:

Successful candidates, prior to hiring, may be required to provide:

  • Background check consent.
  • A clear vulnerable sector screening.
  • BCCNM – Active Registration – and in “good standing “
  • 3 references
  • Completion of our online Praesidium Abuse Training and required Health and Safety training

As Per Facilities Subsector Collective Agreement

$32.84 - $41.35

Compensation:

The target hiring range for this position is $32.84 to $37.10 with a maximum of $41.35.

Placement in the salary range will be based on factors such as market conditions, internal equity, and candidate experience, skills, and qualifications relevant to the role.
