Do you want to be at the forefront of a brand-new initiative that’s set to revolutionize care and support for some of the most vulnerable individuals in our community? If so, we want you to be part of our innovative, compassionate, and dynamic team!
We are opening the doors to a state-of-the-art treatment hub designed to provide comprehensive support and resources that address the root causes of homelessness and addiction. We are currently seeking a 1.0 FTE Support Worker who will be responsible for engaging with clients and interdisciplinary care team members to foster a community-led approach to care at the HART Hub. The Support Worker will provide unique support for clients, advocate for client needs, and assist with relationship building, transportation and accessing health/social services.
One of a network of provincial community health centres, the NorWest Community Health Centres is a community based, non-profit organization funded by Ontario Health, emphasizing access to primary health care and prevention programs and services. The position has a competitive salary, the Health Care of Ontario Pension Plan and an excellent group health benefit package.
QUALIFICATIONS:
- Knowledge of community services and resources, and strong advocacy skills.
- Proven experience with overdose prevention and response, and training/experience with Naloxone administration.
- Experience and knowledge of substance use, harm reduction education, and homelessness. Lived experience is considered an asset in this role.
- Demonstrated ability to de-escalate conflict and strong communication skills.
- Confidence in asserting oneself and maintaining strong boundaries.
- Open, non-judgmental and willing to learn.
- Awareness of and sensitivity to health issues particularly within populations who face barriers to accessing care (under housed, living with mental health challenges or diagnoses, current or past substance use, complex health histories, 2SLGBTQIA+, newcomers, non-insured).
- Proficiency in the use of computers and various software applications.
- A vehicle and a valid driver’s licence is an asset.
