Position Summary:
Reporting to the Clinical Manager of Pharmacy Services, you will be an integral member of the Antimicrobial Stewardship Program (ASP) Team. You will provide direct patient care as well as contribute to the antimicrobial stewardship strategy at Trillium Health Partners under the leadership of the ASP Resource Pharmacists.
The Antimicrobial Stewardship Pharmacist will be responsible for assisting in the optimization of antimicrobial use for the entire organization. You will support further implementation of evidence-based antimicrobial policies and procedures at Trillium Health Partners designed to improve patient outcomes and antimicrobial utilization. You will, as part of the ASP team, optimize patient pharmacotherapy making recommendations to the care teams to enhance outcomes for individual patients.
Key Qualifications:

Baccalaureate Degree in Pharmacy or Pharm D
-
Licensed in Ontario to practice pharmacy (Part A of Register)
-
Strong project management skills and demonstrated ability to navigate and develop relationships with partners across multiple organizations is an asset
-
Completion of hospital pharmacy residency or equivalent experience is preferred; equivalent experience is interpreted as three years of experience in a broad range of pharmacy operational, clinical and/or administrative pharmacy practice
-
Fellowship in Infectious Diseases (or equivalent) an asset
-
Minimum 3 years of recent experience in hospital pharmacy practice
-
Active membership in the Canadian Society of Hospital Pharmacists (CSHP) is an asset
-
Strong leadership, interpersonal, problem solving, organizational and communication skills
-
Experience in providing exceptional patient-centered care
-
Experience in hospital pharmacy systems and distribution systems are an asset
-
Ability to work in a fast-paced environment both independently and cooperatively with a team
-
Demonstrate self-directed learning and continuing education
-
Positive attitude, flexibility to adapt to changing organizational needs and accountability
Responsibilities include but are not limited to :
-
Providing patient-centered care identifying medication related problems and working within the health care team to propose solutions.
-
Support the development, implementation and evaluation of antibiotic management strategies to optimize the prescribing, use and cost effectiveness of these agents.

Develop project work plans in coordination with appropriate internal and external stakeholders to establish project scope, deliverables, accountabilities, schedules, timelines, resources, monitoring, and post project audit/evaluation
-
Collaborate with physicians, nurses, pharmacists, pharmacy technicians and allied health team members regarding project progress and accountabilities
-
Effectively communicate project expectations to team members and stakeholders in a timely and clear fashion
-
Liaise with the clinical manager and ASP Resource Pharmacists on an ongoing basis
-
Plan, schedule and manage project timelines and milestones
-
Participate in pharmacist clinical and distribution activities as needed and support drug use evaluations as needed
-
Assist and evaluate training processes, continuous education and knowledge translation for pharmacy staff
-
Act as a coach and a mentor to clinicians, residents and students to enhance their abilities regarding antimicrobial medication use and clinical care
-
Active involvement in teaching pharmacy, medical and nursing staff and/or other health care professionals and students
-
Promote Pharmacy Services by participating in relevant ASP related committees and professional committees ( ie . PSNs, ASP subcommittee, Infection Control Committee, etc ).

Ability to lead interdisciplinary teams and work with physician groups to affect changes in prescribing
-
Will be expected to support other duties as assigned
