The Opportunity

As a Care Manager at SAHN, you will oversee care for clients living at home—ensuring quality, supporting staff, and adapting care in real time.
You are trusted to:

  • Assess, plan, and adjust care independently

  • Guide and support frontline staff

  • Communicate directly with families and healthcare partners

  • Step in when situations become complex

  • Able to cover clinical nursing duties

This is a hands-on leadership role, not a coordination-only position.

What You Bring

  • RPN or RN designation (CNO in good standing)
  • Strong clinical judgment and confidence in decision-making
  • Experience mentoring or supporting staff
  • Ability to adapt quickly and manage competing priorities
  • Community or home care experience is an asset
