Position Summary:

  • Responsible for development of nutrition care plans and counselling for patients with diabetes.
  • Use a variety of teaching methods, incorporating the principles of adult education and a variety of client engagement or empowerment modalities to facilitate behaviour change, i.e. MI, stages of change
  • Broad understanding of nutrition resources available in the community to support clients with complex needs
  • Functions as part of an inter-disciplinary team and acts as a resource for other health care providers
  • Facilitate group education programs at the DMC
  • Develops nutrition education resources Participates in program planning and evaluation

Key Qualifications:

  • Degree in Human Nutrition and Dietetics combined with an accredited Dietetic Internship program
  • Member in good standing with the College of Dietitians of Ontario
  • Current membership in Dietitians of Canada and Diabetes Educator Sector of Diabetes Canada preferred
  • Certified Diabetes Educator preferred
  • At least 3 years current experience working with clients with diabetes in a health care setting.
  • Good working knowledge of diabetes and other comorbidities through the lifecycle and adult teaching principles
  • Individual counselling and group counselling experience preferred
  • Demonstrated leadership and communication skills
  • Positive work and attendance record essential
  • Reliable, flexible, creative, self-directed professional, able to work effectively and collaboratively within a team environment A second language an asset
